研究目的：

本课题的研究目的是通过因子分析，观察干预前后VAS评分、腰椎ROM、腰椎功能障碍指数ODI等指标的变化，分析基于疼痛分布特征的针刺对慢性非特异性腰痛的临床疗效及生物反应，以期为临床基于疼痛分布特征的针刺治疗慢性非特异性腰痛提供思路及探索。

Objectives of Study：

The purpose of this study is to observe the changes of VAS score, lumbar ROM, lumbar disability index (ODI) before and after the intervention by factor analysis, and analyze the clinical efficacy and biological reaction of acupuncture based on pain distribution characteristics on chronic nonspecific low back pain, so as to provide ideas and exploration for clinical acupuncture treatment of chronic nonspecific low back pain based on pain distribution characteristics.

药物成份或治疗方案详述：

干预方案 1 常规组 常规组进行常规穴位针刺配合TDP灯治疗。 具体操作：让患者采用俯卧位，全身放松，定位穴位，参照普通高等教育中医类规划教材《针灸学》，选取肾俞、大肠俞、腰阳关、环跳及委中，使用碘伏对穴位进行常规消毒，采用单手进针法，选用0.25*40mm规格，左手持针，针对穴位进行针刺治疗。得气后行平补平泻提插捻转，以患者感局部酸麻重胀感为度。每次留针30分钟。留针的同时配合TDP灯照射，灯照时间30分钟，距离皮肤约30cm。以2周为一疗程，周一至周五每天针刺治疗1次，周末休息，治疗10次，共计一疗程。 2 观察组 观察组进行基于疼痛分布特征的针刺配合TDP灯治疗。 干预方法（基于疼痛分布特征的针刺治疗体系）：让患者采用俯卧位，全身放松，定位痛点（①寻找范围：肋缘以下、臀横纹以上及两侧腋中线之间区域。以L4棘突为坐标系原点（0，0）。②测试前准备：与受试者充分沟通，将操作过程告知受试者，并指导受试者使用VAS量表。于每天8:00至17:30在宁德市闽东医院康复医学科同一间检查室测试，室内安静，温度26℃左右。③测试操作：嘱受试者俯卧位，全身放松，测试者手持FDK20压力测痛仪，将探针尖端垂直向下按压皮肤，以F=60N恒力，约0.5kg/cm2/s匀速地施加于测试点上，同时患者对相应测试点进行VAS评分。④评估痛点：共计随机、无序性取点50个，选取5个最痛点。）使用碘伏对痛点进行常规消毒，采用单手进针法，选用0.25*40mm规格，左手持针，针对5个最痛点进行针刺治疗。得气后行平补平泻提插捻转，以患者感局部酸麻重胀感为度。每次留针30分钟。留针的同时配合TDP灯照射，灯照时间30分钟，距离皮肤约30cm。以2周为一疗程，周一至周五每天针刺治疗1次，周末休息，治疗10次，共计一疗程。

Description for medicine or protocol of treatment in detail：

纳入标准：

①符合诊断标准； ②性别不限，年龄18-65岁； ③研究前2周内未接受同类治疗或其他疗法者； ④患者对本次研究知情了解并签署知情同意书。

Inclusion criteria

1. The diagnostic criteria were met; 2. The age ranged from 18 to 65 years old; 3. Those who did not receive similar treatment or other treatments within 2 weeks before the study; 4. Patients were informed of the study and signed informed consent.

排除标准：

①存在腰椎特异性疾病及神经根性疼痛； ②既往有腰部手术史； ③有严重合并心脑血管或肝肾等疾病患者； ④合并严重感染和意识不清及精神障碍患者； ⑤妊娠期、哺乳期及月经期妇女。

Exclusion criteria：

1. There were specific lumbar diseases and nerve root pain; 2. Patients had a history of lumbar surgery; 3. Patients with serious cardiovascular and cerebrovascular diseases or liver and kidney diseases were found; 4. Patients with severe infection, unconsciousness and mental disorders were found; 5. Pregnant, lactating and menstrual women.
