研究目的：

本研究通过对比分析精准改良式椎管扩大成形术与传统椎板成形术治疗多节段脊髓型颈椎病的临床疗效和影像学结果，评价其各自的优缺点，为临床手术方案的选择提供参考。

Objectives of Study：

In this study, the clinical efficacy and imaging results of modified spinal canal enlargement and CLA, which were used to treat multi-segment cervical spondylotic myelopathy, were evaluated by contrast analysis. The advantages and disadvantages of each were evaluated for clinical operation. The choice of the program provides a reference.

药物成份或治疗方案详述：

纳入标准：

（1）具有脊髓型颈椎病的典型症状如肌力、感觉减退，病理征阳性等，经至少6个月的保守治疗无好转者。（2）经MRI确认压迫节段至少大于或等于3个。（3）术前、术后、术后三月、术后6月、术后12月的临床资料完整。

Inclusion criteria

(1) Patients had clear history of symptoms and signs associated with MCSM, on whom conservative treatment had failed; (2) Patients suffered at least three compressed levels confirmed on the magnetic resonance imaging (MRI); (3) The follow-up information are complete.

排除标准：

（1）主要症状表现为根性或轴性症状，而非压迫脊髓引起。（2）合并有后纵韧带骨化、椎管狭窄、黄韧带肥厚、骨折或肿瘤等其他颈椎疾病。（3）接受前后路联合手术，或过往经历颈椎手术者。

Exclusion criteria：

(1) Patients acted mainly as cervical spondylotic radiculopathy (CSR) rather than myelopathy; (2) In addition to cervical disc herniation, other diseases were combined such as ossification of the posterior longitudinal ligament (OPLL), hypertrophic ligamentum flavum (HLF), fractures or tumors, etc.; (3) Patients had a history of any previous cervical spine injury or surgical intervention.

该研究的性质为回顾性研究，其本质是观察性的，研究期间不存在对患者的干预。两种手术方式均是临床上常用的治疗多节段颈椎病的外科手段，行手术的前提是患者及其家属有要求手术的意愿。患者接受何种手术是通过医师团队细致讨论并充分告知患者及其家属手术目的、预期效果及风险，获得其知情同意后的决定，根本目标是为使患者得到最恰当的疾病诊治，故不存在人为干预分组的情况。接受精准改良式椎管扩大成形术的患者分入A组，接受传统颈椎后路椎板单开门术的患者分入B组。
