研究目的：

本试验通过采用多中心、随机、单盲、对照的临床研究方法，纳入符合条件的病例，以“假手法”为对照，“家庭锻炼”为基础治疗，观察五步整肩手法干预慢性肩袖损伤病例后肩关节功能评分、疼痛视觉模拟评分、关节活动度以及健康调查简表的改变量，以及肩关节影像学的变化，明确五步整肩手法对慢性肩袖损伤的作用，为其治疗开辟新的治疗思路。

Objectives of Study：

This experiment by means of multicenter, randomized, single-blind, controlled clinical research method, into the eligible cases, compared to "false", "family exercise" for the foundation treatment, observe the five step of the whole shoulder style intervention in chronic rotator cuff injury cases after shoulder joint function score, visual analog pain score, touting the change of the quantity of motion and health survey, And the changes of shoulder joint imaging, to clarify the role of five-step shoulder manipulation on chronic rotator cuff injury, for its treatment to open up a new treatment idea.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

①年龄在18到65岁之间（包含两端）; ②肩峰前外侧区疼痛或活动时的疼痛，症状持续1至6个月; ③以下4个检查中存在任一阳性结果：当抵抗肱骨外旋或外展时出现疼痛，Jobe测试，Hawkins测试，Neer测试; ④VAS评分≥40mm。

Inclusion criteria

①Be between the ages of 18–65 years; ②Have anterolateral acromial area pain or pain with active shoulder elevation [17] and symptoms lasting 1 to 6 months; ③Have one positive sign in each of the following 4 categories: presence of pain when resisting humeral external rotation or abduction, Jobe Test, Hawkins Test, Neer Test; ④VAS score ≥ 40 mm.

排除标准：

①巨大肩袖撕裂(直径大于5厘米的撕裂)或存在2个或更多肌腱撕裂的临床征象; ②盂肱骨关节炎或其他炎性关节炎; ③冻结肩; ④肩关节骨折或脱位; ⑤颈部或肩部手术史; ⑥严重的骨质疏松症; ⑦神经系统疾病、类风湿性关节炎、肿瘤、帕金森病或其他神经系统疾病患者 ⑧怀孕期妇女。

Exclusion criteria：

①Clinical signs of massive rotator cuff tears (a tear of greater than five centimetres in diameter) or the presence of two or more tendon tears [18, 19]; ②Glenohumeral osteoarthritis (OA) or other inflammatory arthritis; ③Frozen shoulder; ④Fracture or dislocation of the shoulder; ⑤History of neck or shoulder surgery; ⑥Severe osteoporosis; ⑦Neurological disorders, rheumatoid arthritis, tumour, Parkinsons disease or other neurological disorders ⑧Pregnancy.

Eligible participants will be randomly assigned at a ratio of 1:1 to either the FSMT group or the SMT group using a random number generator. The randomization scheme and allocation sequence will be generated by Shanghai Medical Clinical Research Center (Shanghai, China). The randomization sequence will be gener

盲法：

Blinding：

The allocation will be concealed in sequentially numbered sealed opaque envelopes that will be opened by the treating practitioners (blinded to research assistants) at the first intervention. Participants will be unaware of the treatment provided to those in the other group (blinded to participants). To assess the effectiveness of the blinding, the evaluator (research assistants) will answer a question at the week 8 evaluation point related to the allocation ( In your opinion, which intervention did this participant receive? ) . The possible answers are (1) intervention testing a new technique (experimental group); (2) sham intervention (control group); or (3) I have no idea.
