研究目的：

以《中风后认知功能障碍中医诊疗方案》为基础，基于多中心临床研究平台，采用多种方法综合评估该方案在中风病全程治疗中的疗效及康复结局，以获得循证医学证据，进一步制定并优化该方案中的针刺治疗技术。

Objectives of Study：

Based on the Chinese Medicine Diagnosis and Treatment Program for Cognitive Dysfunction after Stroke”, based on a multi-center clinical research platform, multiple methods are used to comprehensively evaluate the curative effect and rehabilitation outcome of the program in the whole course of stroke treatment, so as to obtain evidence-based medical evidence and further formulate and optimize the acupuncture treatment technology in the program.

药物成份或治疗方案详述：

（1）对照组：仅行基础治疗（针对原发病的内科治疗、康复治疗、耳穴治疗、健康教育），其中耳穴治疗隔日一次，一周3次，针刺治疗一周5次。完成3周疗程。 （2）原方案治疗组：在对照组治疗基础上参照2017年中医药管理局发布的《中风后认知功能障碍中医诊疗方案》开展中风后认知功能障碍针灸治疗，一周5次。完成3周疗程。 （3）优化针刺治疗组：在基础治疗及原方案治疗基础上，结合中医康复结局评价及中医状态辨识系统，根据辨证分型加入相应配穴进行体针治疗，一周5次。完成3周疗程。

Description for medicine or protocol of treatment in detail：

(1) Control group: only basic treatment (internal medical treatment, rehabilitation therapy, auricular therapy, health education for primary disease) was performed, including auricular point treatment once every other day, three times a week, and acupuncture treatment five times a week. Three weeks of treatment were completed. (2) Original treatment group: based on the treatment of the control group, acupuncture and moxibustion treatment of cognitive dysfunction after stroke was carried out according to the "Traditional Chinese Medicine diagnosis and treatment program for post-stroke cognitive dysfunction" issued by the Chinese medicine administration in 2017, five times a week. Three weeks of treatment were completed. (3) Optimized acupuncture treatment group: On the basis of basic treatment and original treatment, combined with TCM rehabilitation outcome evaluation and TCM status recognition system, the corresponding acupoints were added for body acupuncture treatment according to the syndrome differentiation, 5 times a week. Three weeks of treatment were completed.

纳入标准：

（1）符合脑卒中西医诊断标准，并经头颅CT或MRI影像学检查证实； （2）45岁≤年龄≤75岁； （3）经MMSE及MoCA 量表筛查确定有认知功能障碍：MMSE<27，MoCA<26； （4）病程在卒中后6个月内； （5）病情稳定，意识清醒； （6）患者本人签署或由其直系亲属代签知情同意书。

Inclusion criteria

(1) Patients who meet the diagnostic criteria of western medicine and traditional Chinese medicine for stroke and was confirmed by head CT or MRI; (2) 45 years old <= age <= 75 years old; (3) Patients whose cognitive dysfunction were identified by MMSE and MoCA; MMSE<27, MoCA<26; (4) Duration within 6 months of stroke; (5) Patients with stable condition and conscious consciousness; (6) The informed consent were signed by the patients or signed by their immediate family members.

排除标准：

（1）存在严重视力或听力障碍、精神障碍等影响认知检查者； （2）发病前有明显智力减退、痴呆病史； （3）合并严重的心、肝、肾疾衰竭，恶性肿瘤，消化道大出血等疾病； （4）妊娠及哺乳期女性； （5）正在参加影响本研究结果评价的其它临床试验者。

Exclusion criteria：

(1) Patients with severe visual or hearing impairment, mental disorder and other factors that affect cognitive examinations; (2) Patients with obvious mental retardation and dementia before onset; (3) Patients with severe heart, liver and kidney failure, malignant tumors, gastrointestinal bleeding and other diseases; (4) Pregnant and lactating women; (5) Patients who are participating in other clinical trials affecting the evaluation of the results of this study.
