研究目的：

1、规范评价颈胸同调手法结合APP功法锻炼对青年颈椎曲度改变的疗效。 2、初步探讨颈椎曲度与颈椎椎间盘早期退变的关系。 3、研究改良传统功法的启项四节操在项目针对人群的接受度及治疗效果，为其推广和普及做基础研究工作。

Objectives of Study：

1. Evaluate of tuina therapy of cervical and thoracic vertebralcombined with APP of exercise on the curative effect of youth cervical curvature change. 2. Discuss the relationship between thecervical curvature and early cervical intervertebral disc degeneration. 3. Investigate the acceptance and treatment effect of the traditional exercise.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

（1）年龄19-40岁，性别不限； （2）具有近1个月内颈椎影像学检查资料；结果显示颈椎退行性改变，经颈椎侧位片测定颈椎生理曲度Borden测量法测定D值＜7mm； （3）主诉为枕部、颈部、肩部疼痛等异常感觉； （4）未经药物、手术及其他治疗者； （5）签署知情同意书，并能够接受连续治疗者。

Inclusion criteria

(1) the age19-40, no gende limited; (2) has less than one month cervical spine examination data;Results show that the cervical degenerative changes, the determination of cervical lateral slice cervical physiological curvature Borden measurement method of D value < 7 mm; (3) the chief complaint are the head, neck, shoulder pain, abnormal feeling; (4) without drugs, surgery, and other treatment; (5) signed informed consent form, and be able to accept continuous treatment.

排除标准：

（1）颈部急性扭伤、落枕、肩周炎和风湿性肌纤维炎等疾病引起的颈肩部疼痛； （2）既往诊断过严重颈椎间盘突出； （3）妊娠或准备妊娠妇女，哺乳期妇女； （4）过敏体质、皮肤敏感者或身体虚弱者； （5）虽符合诊断标准但同时伴有明显的心、脑、肾及消化系统等疾病的患者； （6）严重骨质疏松，颈椎结核、肿瘤，及椎管占位性病变等有手法治疗禁忌症的患者； （7）存在认知功能障碍或交流障碍，不能配合完成量表、治疗或随访者。 （8）有核磁检查禁忌症的患者；

Exclusion criteria：

(1) acute sprains, neck stiffness, periarthritis of shoulder and neck neck shoulder pain caused by diseases such as rheumatoid muscle fiber inflammation; (2) the previous diagnosis of severe cervical intervertebral disc herniation; (3) pregnancy or ready to pregnant women, nursing mothers. (4) allergic constitution, sensitive skin or weak people; (5) diagnosis standard but at the same time companion has apparent heart, brain, kidney and digestive system disease patients; (6) severe osteoporosis, cervical vertebra tuberculosis, tumour, contraindications and vertebral canal space-occupying lesions with manipulation treatment of patients; (7) or communication disorders, cognitive dysfunction can't cooperate to complete inventory, treatment, or finish the visitor. (8) patients with nuclear magnetic inspection contraindication;

采用区组随机，对所有病例进行随机分组。借助SAS统计软件PROC PLAN过程语句，给定种子数，产生150例受试者所接受处理（牵引组和锻炼组和手法+锻炼组）的随机安排（即随机编码表）。参照随机编码表按照患者入组顺序比对进行分组，编号001-030入牵引组，编号031-090入手法+锻炼组，编号091-150入锻炼组。

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1）研究者需及时、完整、准确地将数据记录到病例记录表中；2）由专业数据录入人员进行二次录入；3）数据管理员与主要研究者对录入数据进行数据范围及逻辑检查；4）原始病例报告表归档进行保存，电子数据分类进行多重备份。
