研究目的：

明确餐后采取何种体位有利于预防住院老年原发性高血压患者PPH的发生，降低用餐前后的血压变异率。

Objectives of Study：

Determining which kind of after-meal-positionsis favorable forpreventing PPH in hospitalized elderly adults with primary hypertension, reducing blood pressure variability before and after meals.

药物成份或治疗方案详述：

平卧位组：入组患者在研究当天早晨取坐位在病房内进餐，早餐时间为上午7:00-7:30，食物由中日友好医院食堂统一配送，以粥和馒头为主。从患者第一口饭开始计时，要求患者在30分钟内进食早餐完毕，在测试当天均在干预结束后按照治疗方案服用长效降压药。患者在早餐后采取平卧位，对卧位的具体姿势如侧卧、平躺等不做要求。计时从第一口饭开始至120分钟止。 半坐卧位组： 入组患者在研究当天早晨取坐位在病房内进餐，早餐时间为上午7:00-7:30，食物由中日友好医院食堂统一配送，以粥和馒头为主。从患者第一口饭开始计时，要求患者在30分钟内进食早餐完毕，在测试当天均在干预结束后按照治疗方案服用长效降压药。患者在早餐后即采取45°半坐卧位。计时从第一口饭开始至120分钟止。 日常活动组：入组患者在研究当天早晨取坐位在病房内进餐，早餐时间为上午7:00-7:30，食物由中日友好医院食堂统一配送，以粥和馒头为主。从患者第一口饭开始计时，要求患者在30分钟内进食早餐完毕，在测试当天均在干预结束后按照治疗方案服用长效降压药。患者在早餐后进行日常活动，但期间不能取平卧位及半坐卧位姿势超过30分钟，不得离开所在病区，计时从第一口饭开始至120分钟止。

Description for medicine or protocol of treatment in detail：

纳入标准：

1. 符合《中国高血压防治指南2018年修订版》提出的高血压诊断标准，确诊为原发性高血压； 2. 年龄≥60岁； 3. 意识清楚，有独立行为能力； 4. 服用长效降压药物； 5. 干预前连续3天监测早餐前后血压变化，PPH发生次数在2次及以上； 6. 同意参加本研究。

Inclusion criteria

1. It conforms to the diagnostic criteria of hypertension proposed by Revised Guidelines for the Prevention and Treatment of Hypertension in China in 2018and is diagnosed as primary hypertension. 2. Age≥60 years. 3. Consciousnessindependent behavior ability. 4. Take long-acting anti-hypertensive drugs. 5. Blood pressure changes before and after breakfast are monitored for 3 consecutive days before intervention. The occurrence of PPH is more than 2 times. 6. Agree to participate in this study.

排除标准：

1. 存在晨峰高血压（晨起后2 小时内收缩压的平均值减去夜间睡眠时收缩压最低值的差值≥35mmHg）； 2. 处于高血压急症或亚急症状态； 3. 近6个月内发生急性心脑血管事件； 4. 近6个月内有严重外伤及手术史； 5. 由于各种原因不能接受常规饮食； 6. 近1个月内使用硝酸酯类、地高辛、奥利司他、阿卡波糖、奥曲肽、瓜尔胶、3，4-DL-苏二羟、咖啡因或其他精神类药物； 7. 患有帕金森综合征、多系统萎缩、阿尔茨海默病。

Exclusion criteria：

1. Morning peak hypertension (The mean of systolic blood pressure within 2 hours after morning minus the lowest systolic blood pressure during night sleep >=35 mmHg). 2. In the state of hypertensive emergency or sub-acute symptoms. 3. Acute cardiovascular and cerebrovascular events occurred within months. 4. Severe trauma and surgical history within 6 months. 5. Routine diet is unacceptable for various reasons. 6. Using nitratesdigoxinorlistatacarboseoctreotideguar gum34-DL-SODcaffeine or other psychotropic drugs within one month. 7. Suffering from Parkinson's syndromemultiple system atrophy, or Alzheimer's disease.

采用随机数字表法产生随机数字，从第三行第5列开始取值，每两位数为一个数值，将所得随机数字除以3后，事先约定余数为0时进入日常活动组，余数为1时进入半坐位组，余数为2时进入卧位组，三组分别达到24人时即止。将标有“ 0 ”、“ 1 ”、“2”数字的纸条装入不透明信封并由研究专人保管。患者按照入院先后顺序依次打开相应序号的信封，按照相应的数字进行分组。
