本研究通过观察中医背景下的共同照护模式对接受规律管理的2型糖尿病患者血糖及代谢指标的影响，借助量表评估该模式对患者自我管理水平的影响，评价引入中医特色的共同照护模式对糖尿病管理的意义，以期借助中医药优势优化当前的糖尿病管理模式，拓展中医“治未病”理论在糖尿病管理中的应用。

Objectives of Study：

By observing the effect of the TCM diabetes shared care on blood glucose and metabolic indicators in patients with type 2 diabetes who received regular management, and using the scale to evaluate the effect of the mode on the self-management level of patients, the effect of the co-care mode with the characteristics of traditional Chinese medicine was evaluated on the The significance of diabetes management, in order to optimize the current diabetes management model with the advantages of traditional Chinese medicine, and expand the application of traditional Chinese medicine "preventive treatment" theory in diabetes management.

药物成份或治疗方案详述：

中医糖尿病共同照护模式组:采用中医糖尿病共同照护模式：在线下门诊，中医内分泌科医生进行中医证候体质辨识并开具专业中西药处方，同时卫教师负责药物评估和风险教育等，营养师负责营养评估、营养教育和中医食疗药膳指导，运动康复师进行日常患者运动评估指导及太极拳、五禽戏、八段锦等相关中医传统功法的指导，其他护理人员协助进行足部筛查和足部日常照护的指导。除此之外，眼科、皮肤科、肾内科及心血管内科等多科室专业人士配合，进行并发症筛查。收集保存患者看诊、检验检查、卫教每个环节的重要数据，患者在提醒下规范进行每三个月门诊回访，保证完整的病程追踪。 在患者居家时，共同照护模式以线上的院外服务进行衔接。在患者知情同意且保证医疗信息安全的情况下，通过智能血糖仪设备与手机APP链接，利用物联网及移动互联网获取血糖监测信息、测量数据、饮食及运动数据，照护师及时回答患者在家自我管理过程中的疑问，遇到特殊情况及时反馈给内分泌专科医生。 非中医糖尿病共同照护模式组:采用常规护理模式：医护人员根据患者心理水平及对疾病的重视程度开展针对性的健康教育。叮嘱患者规范用药，与患者主治医生保持紧密联系，控制用药时间及剂量，保证用药规范性。每天定时检测身体情况，发现异常立即报告主治医师，采取针对性疾病干预，保证患者在第一时间得到最佳护理管理方案。

纳入标准：

符合糖尿病的诊断；同意加入中医糖尿病共同照护管理，签署知情同意书；能规律复诊及使用手机App、智能血糖仪设备的患者。

Inclusion criteria

Patients who meet the diagnosis of diabetes; agree to join the joint care and management of diabetes in traditional Chinese medicine, sign the informed consent form; patients who can regularly follow up and use mobile app and smart blood glucose meter equipment.

排除标准：

非糖尿病患者；不同意加入中医糖尿病共同照护管理；无能力使用手机App、智能血糖仪设备的患者。

Exclusion criteria：

Non-diabetic patients; patients who do not agree to join the joint care and management of diabetes in traditional Chinese medicine; patients who are unable to use mobile apps and smart blood glucose meter devices.
