研究目的：

本研究旨在探讨健脾滋阴法联合个体化肠内营养支持对肺脾两虚型COPD缓解期患者临床症状、生存质量和营养状态的影响。

Objectives of Study：

The purpose of this study was to investigate the effect of invigorating spleen and nourishing Yin combined with individualized enteral nutritional support on clinical symptoms, quality of life and nutritional status of COPD patients with lung and spleen deficiency.

药物成份或治疗方案详述：

将存在营养风险的肺脾两虚型COPD缓解期患者随机分为对照组（基础治疗）、基础治疗联合营养组、中药治疗组和中药联合营养组。对照组患者给予沙美特罗/替卡松每吸50μg/250μg，每日两次+茶碱缓释片0.1g，每日两次，另外，使用沙丁胺醇缓解气管痉挛，抗生素或乙酰半胱氨酸化痰等基础治疗。基础治疗联合肠内营养组：在基础治疗同时联合口服肠内营养剂及给予个体化肠内营养指导方案；中药治疗组：患者在基础对症治疗的基础上，加服健脾滋阴法拟方；中药联合营养组：在基础治疗+健脾滋阴法拟方治疗的基础上，口服肠内营养剂及给予个体化肠内营养指导方案。 健脾滋阴法拟方组分：太子参15g，麦冬15g，生地黄10g，玉竹15g，白术15g，茯苓30g，陈皮5g，生黄芪15g，生甘草6g，山药30g，女贞子15g，焦三仙各10g。剂型：配方颗粒剂，每次1袋，温开水200ml冲服，一日2次，共治疗3个月。 个体化肠内营养支持方案如下：根据患者白蛋白、BMI、前白蛋白、总淋巴细胞计数、每日活动量，制定肠内营养计划和三餐标准。根据患者的具体情况选择基础制剂以及适当的添加组分。另外，食物对气体交换的影响，尽量减低饮食的耗氧气量，选择呼吸商低的食品，以免产生过多二氧化碳发生呼吸性酸中毒。疗程3个月。连续治疗3个月后，检测各项指标。

Description for medicine or protocol of treatment in detail：

The COPD patients with deficiency of both lung and spleen were randomly divided into four groups: Control Group (basic treatment). Basic Treatment Combined with Nutrition Group, Traditional Chinese Medicine Treatment Group and traditional Chinese Medicine Combined Nutrition Group. Patients in the control group will be given Salmeterol Xinafoate and Fluticasone Propionate Powder for Inhalation 50ug/250ug twice daily,Oral Theophylline sustained release tablets 0.1g,twice daily, plus Salbutamol, plus basic treatment such as bronchospasm relief, antibiotics or L-cysteine. Basic Treatment combined with Nutrition Group: will be given oral enteral nutrition and individual enteral nutrition guidance program and basic treatment;Chinese Medicine Treatment Group: Patients on the basis of basic symptomatic treatment, plus taking Jianpi Ziyin formula; Chinese Medicine Combined Nutrition Group: On the basis of basic treatment,wil be given Jianpi Ziyin formula, oral enteral nutrition and were given the individual enteral nutrition guidance program. The prescription components of Jianpi Ziyin formula: Radix pseudostellariae 15g, Radix ophiopogonis 15g, Radix rehmanniae 10g, Rhizoma Polygoni Multiflori 15g, rhizoma atractylodis 30g, dried Tangerine Peel 5g, radix astragali 15g, Radix glycyrrhizae 6g, Rhizoma dioscoreae 30g, fructus Ligustri Lucidi 15g, each 10g. Dosage Form: Formula Granules. 1 bag each time is diluted with 200ml warm boiled water.Frequency: 2 times a day. course of treatment : 3 months. The individualized enteral nutrition support program is as follows: according to patients'albumin, BMI, Prealbumin, total lymphocyte count, daily activity, establish enteral nutrition plan and three meal standard. According to the specific conditions of patients to choose the basic preparation and the appropriate added components. In addition, the impact of food on gas exchange, as far as possible to reduce dietary oxygen consumption, choose low consumption of food, so as not to produce too much carbon dioxide Respiratory acidosis. Three months of treatment. After 3 months of continuous treatment, all the indexes were measured.

纳入标准：

COPD缓解期患者；肺功能分级为Ⅱ-Ⅲ级者；年龄40-80岁；临床资料完整；纳入前14天未服中药治疗；4周内未参与其他具有干预作用的临床研究。

Inclusion criteria

1. COPD patients in remission stage; 2. patients with Pulmonary Function Grade ii-iii; 3. aged 40-80 years; 4. Complete Clinical Data; not taking traditional Chinese medicine for 14 days before inclusion; 5. not participating in other clinical intervention studies within 4 weeks.

排除标准：

近2周曾发生急性呼吸道感染者；近1个月全身使用过糖皮质激素者；由支气管扩张、肺间质纤维化等引起或其他具有特征病理表现的气流受限性疾病；合并肺结核、恶性肿瘤、严重肝肾功能损伤、造血系统疾病、精神疾病者；妊娠或哺乳期女性；依从性差，不愿意配合和完全不能交流者；对本研究用药过敏或不耐受者。

Exclusion criteria：

1. Patients who did not meet the inclusion criteria; 2. Those who had developed acute respiratory tract infection in the last 2 weeks; 3. Those who had used glucocorticoid in the last 1 month; 4. Patients with pulmonary tuberculosis, malignant tumor, severe injury of liver and kidney function, hematopoiesis disease and mental disease; 5. Pregnant or lactating women; 6. Poor compliance, unwillingness to cooperate and complete inability to communicate; 7. Allergic or intolerant to the drug used in this study.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

研究者根据受试者的原始观察记录，将数据及时、清晰、完整的记入病例观察表中，采用相应的数据系统由双人双机录入，并对数据库进行2遍比较，电子数据文件分类保存，并有多个备份保留于不同的磁盘中，妥善保管，防止损坏。
