研究目的：

冠状动脉介入治疗已成为冠状动脉疾病诊断和治疗的重要手段，然而，高达63%的冠脉介入患者在术前存在焦虑状态，它不仅是一种情绪障碍，还导致心动过速、高血压和动脉血管收缩，可能促发或加重冠心病的症状，但术前焦虑还尚未得到充分重视、检测和治疗。现虽可使用药物缓解焦虑，但易导致其他不良反应发生。近年来，中医疗法在临床实践和辅助治疗中的优势日益明显。因此，本项目拟着眼于探讨冠状动脉介入术前焦虑的发生与缓解措施，在术前对干预组采用中医穴位贴敷及五行音乐疗法，对照组常规治疗护理，从而比较两组对冠脉介入术前焦虑及疾病预后的影响，为进一步将冠脉介入术前焦虑症患者进行规范化管理及采取更加针对性的护理措施提供坚实理论基础。

Objectives of Study：

Coronary intervention therapy has become an important means of diagnosis and treatment of coronary artery disease, however, up to 63% of patients with coronary intervention have anxiety before surgery, which is not only a mood disorder, but also leads to tachycardia, hypertension and arterial vasoconstriction, which may promote or aggravate the symptoms of coronary heart disease, but preoperative anxiety has not yet been fully valued, detected and treated. Although drugs can be used to relieve anxiety, it is easy to cause other adverse reactions. In recent years, the advantages of TCM in clinical practice and adjuvant therapy have become increasingly obvious. Therefore, this project aims to focus on the occurrence and mitigation measures of preoperative anxiety in coronary intervention, and to use TCM acupuncture point patches and five elements music therapy and conventional treatment and nursing care in the control group before surgery, so as to compare the effects of the two groups on preoperative anxiety and disease prognosis of coronary intervention, and provide a solid theoretical basis for further standardized management of patients with preoperative coronary interventional anxiety disorder and more targeted nursing measures.

纳入标准：

1.我院心血管病中心行择期冠状动脉造影术的患者； 2.年龄18-65岁，意识良好，有独立行为能力； 3.能接受穴位敷贴联合五行音乐疗法且依从性好； 4.对本研究知情，自愿参与，签署知情同意书；

Inclusion criteria

1. Patients undergoing elective coronary angiography at the Cardiovascular Disease Center of our hospital; 2. Age 18-65 years old, good consciousness, independent ability; 3. Can accept acupuncture point patch combined with five elements music therapy and good compliance; 4. Be informed of the study, voluntarily participate, and sign an informed consent form;

排除标准：

1.未行冠状动脉介入治疗患者； 2.患有严重的肝、肾、心、肺、脑等功能不全者或有严重并发症者； 3.有家族精神病或遗传病史者或近3天服用抗焦虑、镇静药患者； 4.重度抑郁患者，尤其是存在自残、自杀倾向和行为者； 5.有智力缺陷、不能配合治疗及中途放弃放疗的患者； 6.无法自主配合进行汉密顿焦虑量表（HAMA）评分评估的患者； 7.妊娠或哺乳期妇女； 8.听力障碍患者； 穴位敷贴部位皮肤破损者或敷贴过敏者。

Exclusion criteria：

1. Patients who have not undergone coronary intervention; 2. Those with severe liver, kidney, heart, lung, brain and other insufficiency or serious complications; 3. Patients with family history of psychosis or genetic diseases or patients who have taken anti-anxiety and sedative drugs in the past 3 days; 4. Patients with severe depression, especially those with self-harm, suicidal tendencies and behaviors; 5. Patients with mental retardation, inability to cooperate with treatment and abandonment of radiotherapy in the middle of the course; 6. Patients who are unable to independently cooperate with the Hamilton Anxiety Scale (HAMA) score assessment; 7. Pregnant or lactating women; 8. Hearing impaired patients; Those who have broken skin at the site of acupuncture point application or who are allergic to applying.

干预组患者在对照组的常规护理基础上加穴位贴敷联合五行音乐疗法，①穴位：取膻中、双侧的内关、三阴交和足三里。使用复方辣椒贴准确贴于穴位上，复方辣椒贴的水杨酸具有良好的消炎作用，辣椒素和醋酸生育酚有促进人体血液循环之效，樟脑和薄荷醇能够缓解局部疼痛的作用。根据敷贴说明12小时更换一次，注意观察有无过敏。穴位贴敷由两名经中医师统一培训、考核，经验丰富的临床护士进行，以减少对试验结果的影响；②五行音乐治疗：术前由专业护士详细介绍治疗目的和方法。根据中医整体理论和五音疗法辨证理论，采用宫调（Do）式乐曲《满江红》、《闲居吟》、《小白杨》和羽调（La）式乐曲《船歌》、《绣红旗》、《红梅赞》等。以上音乐均选自中医五行音乐曲库。将所有符合曲目分别装载到随身听中。嘱患者全身放松、闭目养神，音量不超过40-60 dB，以患者自我感觉舒适为度，尽可能保持环境安静，禁止探视和电话干扰等；在播放过程中可以依据患者爱好切换同一类乐曲的不同曲目，但不宜长时间单曲循环播放，避免久听生厌；五行音乐治疗术前每日早10:00，晚9:00，各听30min。手术当天停止音乐疗法。③干预时间：入组后干预至手术前一天。
