研究目的：

通过探讨社区建立中医全科医师参与的PCMH模式的可行性及有效性，丰富我国家医签约服务的形式；研究小儿功能性便秘在社区的诊疗流程，制定该病在PCMH模式下的诊疗服务规范，提高儿童社区常见病诊疗能力，为逐步实现儿童常见病基层首诊、推动分级诊疗奠定基础。

Objectives of Study：

Though exploring the feasibility and effectiveness of establishing PCMH model with the participation of the TCM（Traditional Chinese Medicine） General Practitioners in the community, the practice of contracted medical care in China will be popularized. This program aims to study the diagnosis and treatment process of children's functional constipation in the community, to formulate the diagnosis and treatment service standard of the disease under the PCMH modelto improve the diagnosis and treatment ability of common diseases in the community of children, and to lay a foundation for gradually realizing the early diagnosis of common diseases in the community of children and to promote the hierarchical diagnosis and treatment.

药物成份或治疗方案详述：

将中医全科医师、儿科医师、公卫医师、社区护士、心理咨询师、营养师等成员共同组成服务团队，以团队的方式对具有功能性便秘的患儿进行连续性管理，包括中医治疗、西医治疗、健康教育、健康咨询等服务。家长参与到此模式当中，组建“家长学校”，通过现场教学及多媒体手段学习相关知识，掌握相应中医适宜技术（包括小儿推拿、捏积等手法），并进行过程考核。

Description for medicine or protocol of treatment in detail：

The service team will be composed of TCM General Practitioners, Pediatricians, Public Health Physicians, Community Nurses, Psychological Consultants, Nutritionists and other members. In a medical team, the routine care of children with functional constipation will be carried out, including TCM treatment, Western Medicine treatment, health education, health consultation and other services. In this mode, parents participate in the establishment of a "parent school" to learn relevant knowledge through on-site teaching and multimedia means, they will master the appropriate technologies of TCM (including child massage, kneading and other techniques), and they will complete the process assessment.

纳入标准：

0-14岁儿童，确诊功能性便秘。 诊断标准参照儿童功能性胃肠病罗马罗马IV标准：至少符合以下2项条件,持续时间达1个月以上： （1）每周排便≤2次; （2）大量粪便潴留史; （3）有排便疼痛和排便费力史; （4）排粗大粪便史; （5）直肠内存在有大量粪便团块。对于接受排便训练的儿童,以下条件也作为选项： （6）能控制排便后每周至少出现1次大便失禁; （7）粗大粪便曾堵塞抽水马桶。 注：对于9个月以内的婴儿，在除外其他健康问题的前提下，用力排便的时间超过10分钟的应考虑婴儿排便困难。

Inclusion criteria

Children were diagnosed with FC if they met Rome IV criteria: at least 1 month of two or more of the following criteria: (1) two or fewer defecations per week; (2) history of pain/hard bowel movements; (3) history of large-diameter stools; (4) presence of a large fecal mass in the rectum; (5) history of excessive stool retention. In toilet-trained children, additional criteria include at least 1 episode/week of fecal incontinence and history of large-diameter stools that may obstruct the toilet. Infants under 9 months of age, excluding other health problems, difficulty in defecating should be considered if forced defecation lasts longer than 10 minutes.

排除标准：

确诊的器质性疾病引起的便秘、急性感染性疾病、皮肤病、对通便药物过敏、长期使用诱发便秘的药物、精神疾病。

Exclusion criteria：

Constipation due to diagnosed organic diseases, acute infectious diseases, skin diseases, allergy to laxative drugs, long-term use of drugs that induce constipation, and mental diseases.
