研究目的：

针灸治疗卒中后吞咽障碍方法多样，寻找更加有效的针灸治疗方法，进一步提高针灸治疗的有效性及安全性，为临床提供实践指导，将针灸治疗卒中后吞咽障碍推广、运用至更多、更广的范围有着重要的临床意义。“项七针”是全国老中医药专家学术经验继承工作指导老师单秋华教授腧穴配伍处方的经验总结，采用局部和左右配穴方法，取风府及双侧风池、天柱、完骨穴等七个穴位进行施针。依据“气街”与“四海”理论，位于颈项部的项七针针法能够疏通经络、调畅气血、清脑益髓，达到改善神经功能，促进局部血液循环，改善气血运行，加速卒中后受损组织的恢复的作用，来进一步减少吞咽障碍的发生或改善吞咽障碍。将项七针针法应用于临床，可给卒中后吞咽障碍的治疗提供了新选择与新方法，提高临床疗效。 静息态功能磁共振成像（rs-fMRI）是指在保持清醒、闭眼、平静呼吸且不进行任何思维活动和自主运动的静息状态下进行数据采集的一种磁共振成像技术，其采集的BOLD信号反映了脑组织神经元的自发活动。受试者无需接受任何刺激，可避免由于实验设计或被试执行任务时的差异导致实验结果不可靠，具有较高的可重复性和实用性。目前功能磁共振研究已广泛地应用于脑卒中治疗的机制研究中，但是关于针刺治疗的中枢机制尚不明确，其中也包括吞咽障碍的研究。本研究拟通过功能磁共振技术结合项七针疗法探寻针刺治疗卒中后吞咽障碍其中的中枢机制。

Objectives of Study：

Various acupuncture methods are available for the treatment of post-stroke dysphagia. Identifying more effective acupuncture approaches to further enhance the efficacy and safety of treatment holds significant clinical value. Such advancements can provide practical guidance for clinical applications and facilitate the wider promotion and application of acupuncture in treating post-stroke dysphagia. The "Xiangqi Acupuncture" technique is a prescription derived from the experience of Professor Dan Qiuhua a mentor in the National Program for the Academic Inheritance of Traditional Chinese Medicine Experts. This technique utilizes a combination of local and bilateral acupoint selection involving seven acupoints: Fengfu (GV16) bilateral Fengchi (GB20) Tianzhu (BL10) and Wangu (GB12). Based on the theories of "Qi Street" and "Four Seas" this cervical-region-focused acupuncture method aims to unblock the meridians regulate qi and blood clear the mind and nourish the marrow. Its therapeutic effects include improving neurological functions promoting local blood circulation enhancing the flow of qi and blood and accelerating the recovery of damaged tissues following a stroke. These effects collectively reduce the occurrence of dysphagia or improve swallowing function. Applying the Xiangqi Acupuncture technique in clinical practice provides a novel option for the treatment of post-stroke dysphagia significantly improving clinical outcomes. Moreover combining this technique with functional magnetic resonance imaging (fMRI) offers an opportunity to investigate the central mechanisms underlying the therapeutic effects of acupuncture in treating post-stroke dysphagia.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

（1）符合脑卒中诊断标准患者； （2）经过吞咽造影检查（VFSS）显示存在吞咽障碍的患者； （3）目前饮食受限制，洼田饮水评分Ⅱ级以上； （4）简易精神状态检查（MMSE）评分达到24分以上，能理解和执行治疗人员的简单指令，认知能力尚可，能够配合并愿意检查和治疗者； （5）年龄在35～70岁之间，性别不限； （6）发病时间能够确定且在90天内； （7）神志清楚，生命体征平稳； （8）患者无严重并发症，能接受针灸治疗且依从性好； （9）经患者或家属同意，自愿参与此项研究，并由本人或其合法亲属签署知情同意书。

Inclusion criteria

1. Patients meeting the diagnostic criteria for stroke. 2. Patients diagnosed with dysphagia through videofluoroscopic swallowing study (VFSS). 3. Current dietary intake is restricted with a Watian drinking water test score of Grade II or above. 4. Mini-Mental State Examination (MMSE) score of 24 or higher demonstrating sufficient cognitive ability to understand and follow simple instructions from the treatment provider. 5. Aged between 35 and 70 years regardless of gender. 6. Onset of stroke can be determined and occurred within 90 days. 7. Clear consciousness and stable vital signs. 8. No severe complications with good tolerance and compliance for acupuncture treatment. 9. Patients or their families agree to voluntarily participate in the study with the patient or their legal representative signing the informed consent form.

排除标准：

（1）非脑卒中原因导致的吞咽障碍； （2）患有影响吞咽功能的其他疾病（如神经肌肉疾病、鼻咽癌、头颈癌等）； （3）妊娠或者哺乳期妇女； （4）合并心、肝、肾、造血系统和内分泌系统等严重原发性慢性疾病者； （5）患有严重痴呆、精神疾病等言语理解有严重障碍不能配合治疗人员指令者； （6）患有各种出血倾向疾病者； （7）近1个月内接受其他类型吞咽障碍治疗者。

Exclusion criteria：

1. Dysphagia caused by conditions other than stroke. 2. Patients with other diseases affecting swallowing function (e.g. neuromuscular diseases nasopharyngeal cancer head and neck cancer). 3. Pregnant or breastfeeding women. 4. Patients with severe primary chronic diseases involving the heart liver kidneys hematopoietic system or endocrine system. 5. Patients with severe dementia mental disorders or severe speech comprehension impairments who are unable to follow instructions from treatment personnel. 6. Patients with diseases associated with bleeding tendencies. 7. Patients who have received other types of treatment for dysphagia within the past month.

本研究为随机对照研究，采用随机、单盲、平行对照的方法进行。通过 SPSS25.0 统计软件，产生 74 个随机数，按 1:1 比例随机分成对照组和治疗组，将随机数与其组别制作成数字卡片，内容包括随机数字、组别、治疗方法，将信封密封并编号，信封编号与随机顺序相同。让符合病例选取标准的患者按照就诊顺序，抽取信封，按照信封中卡片的信息，进入相应组别，进行对应治疗及评估，同时将分配序列隐藏。

Randomization Procedure (please state who generates the random number sequence and by what method)：

This study is a randomized controlled trial using a randomized single-blind parallel control method. A total of 74 random numbers were generated using SPSS 25.0 statistical software and randomly divided into control and treatment groups at a 1:1 ratio. Random numbers group assignments and treatment methods were written on numbered cards sealed in envelopes and numbered in sequence corresponding to the randomization order. Patients meeting the inclusion criteria were enrolled according to their order of consultation selected an envelope and were assigned to the respective group and treatment based on the card's information. The allocation sequence was concealed.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

数据采集用CRF表，研究者根据受试者的原始观察记录，将数据及时、完整、正确、清晰的载入病例报告表，录入采用相应的数据库系统双人双机录入，之后对数据库进行两边比对。电子数据文件分类保存，并有多个备份保存于不同磁盘或记录介质上，妥善保存，防止损坏。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data collection was performed using Case Report Forms (CRFs). Researchers transferred data from the subjects original observation records to the CRFs in a timely complete accurate and clear manner. Data entry was conducted using a double-entry method with two individuals and two separate systems. The database entries were then cross-checked for consistency. Electronic data files were categorized and saved with multiple backups stored on different disks or recording media to ensure proper preservation and prevent damage.
