经皮穴位电刺激足三里及内关穴促进急诊骨折患者胃排空的有效性

Objectives of Study：

The effectiveness of transcutaneous electrical stimulation of the Zusanli and Neiguan acupoints in promoting gastric emptying in emergency fracture patients.

药物成份或治疗方案详述：

单侧足三里、内关穴分别放置电极片，以频率 5Hz ，电刺激强度以患者可耐受最大电流为宜，具体先测定患者刚开始感到轻微刺激的刺激强度为 其“感觉域”，例如电极片接触皮肤的面积是 4*4cm 时，感觉域约为 3～5mA，根据个人耐受能力不同继续缓慢调节电流强度至其感觉域的 2 倍甚至３倍（即6～15mA）时，患者有麻、刺感或局部肌肉出现轻微颤动，即为 TEAS 治疗效果最佳的刺激强度，电刺激持续60min。

Description for medicine or protocol of treatment in detail：

Electrode pads are placed on one side of the Zusanli (ST36) and Neiguan (PC6) acupoints with a stimulation frequency of 5 Hz. The intensity of the electrical stimulation should be set to the maximum current that the patient can tolerate. Initially the stimulation intensity should be determined at the threshold where the patient first feels a slight sensation referred to as their "perceptual range." For example if the area of the electrode pad in contact with the skin is 4 cm x 4 cm the perceptual range is approximately 3-5 mA. Depending on individual tolerance the current intensity can be gradually adjusted to 2 to 3 times the perceptual range (i.e. 6-15 mA) until the patient experiences numbness tingling or slight tremors in the local muscles. This range represents the optimal stimulation intensity for TEAS treatment and the electrical stimulation should be applied for 60 minutes.

纳入标准：

(1) 拟行急诊手术的骨折患者；(2) ASAⅠ-Ⅲ级；(3) 饱胃

Inclusion criteria

Patients with fractures scheduled for emergency surgery; ASA Class I-III; Full stomach

排除标准：

(1)年龄<18岁或>60岁；(2)体重指数<18 kg/m²或>30 kg/m²；(3)无法获得胃超声清晰影像、无法行半卧位；(4)原有胃排空延迟的情况-糖尿病、孕产妇、使用阿片类药物；(5)与胃排空延迟相关的消化道疾病者(胃轻瘫、功能性消化不良）；(6)既往胃肠道手术史；(7)足三里和内关穴位局部及所在经络有手术史者；(8)健侧穴位有皮肤损伤；(9)健侧四肢神经损伤史者；(10)合并严重的系统性疾病；(11)脊柱骨折或脊髓损伤患者；(12)骨折合并其他脏器严重损伤（如腹部损伤、颅脑损伤等）；(13)法律规定的残疾患者（盲，聋，哑，智力障碍，精神障碍)；(14)怀疑或确有酒精、药物滥用病史；(15)需立即行急诊手术导致研究时间不足（4h)；(16)患者或家属不同意参与本研究

Exclusion criteria：

Age < 18 years or > 60 years; Body mass index (BMI) < 18 kg/m² or > 30 kg/m²; Inability to obtain clear gastric ultrasound images or unable to maintain a semi-reclining position; Pre-existing delayed gastric emptying conditionssuch as diabetes pregnancy and opioid use; Gastrointestinal diseases associated with delayed gastric emptying (gastroparesis functional dyspepsia); History of previous gastrointestinal surgery; History of surgery at Zusanli (ST36) or Neiguan (PC6) acupoints or along the respective meridians; Skin lesions at the acupoints on the healthy side; History of nerve injury in the limbs on the healthy side; Presence of severe systemic diseases; Patients with spinal fractures or spinal cord injuries; Fractures with severe injuries to other organs (e.g. abdominal injuries cranial injuries etc.); Patients with disabilities as defined by law (blindness deafness mute intellectual disabilities mental disorders); Suspicion of or confirmed history of alcohol or drug abuse; Need for immediate emergency surgery resulting in insufficient study time (less than 4 hours); Patient or family member does not consent to participate in this study.
