研究目的：

为提升中国基层医疗服务发展水平，本研究拟与基层医疗卫生机构开展合作共创，发挥中医服务与客观化传承两方面的作用。针对基层医生工作场景，通过对患者舌、脉等人体健康信号客观化采集，形成针对患者的疗效数字化跟踪服务体系。

Objectives of Study：

In order to improve the services of primary medical care in China, this study intends to cooperate with primary medical and sanitary institutions, and plays a role of TCM service and objectified inheritance. In the primary medical scene, this study will objectively collect the patient's tongue, pulse and other human physiological signals to form a digital tracking service system for therapeutic effect.

纳入标准：

（1）本研究对年龄无要求，能配合完成本研究，并自愿签署知情同意书者都可入选。 （2）18岁以下未成年，先取得未成年人的父母或其他监护人的同意并签署知情同意书。

Inclusion criteria

(1) There is no age requirement for this study, and those who can cooperate with the completion of this study and voluntarily sign the informed consent form are eligible for inclusion. (2) Minors under the age of 18 shall first obtain the consent of the minor's parents or other guardians and sign the informed consent.

本研究受试者为社区卫生服务中心就诊的患者，不人为的对患者入组条件、年龄以及用药方案等进行限制。拍照及脉搏测量由社区卫生服务中心负责采集数据的工作人员操作，不做其他干预，对人体安全无创。我们的目标是收集广泛的数据，以提高研究的代表性和普遍性。

Randomization Procedure (please state who generates the random number sequence and by what method)：

The subjects of this study were patients seeking treatment in community health service centers, and the enrollment conditions, age and medication regimen of patients were not artificially restricted. The photography and pulse measurement are operated by the staff responsible for data collection in the community health service center, without any other intervention, which are safe and non-invasive to the human body. Our goal is to collect a wide range of data to improve the representativeness and generality of the study.
