研究目的：

评价益气活血法（麝香通心滴丸）治疗气虚血瘀型蒽环类药物相关心肌损伤患者的疗效与安全性，并评估其远期心脏保护作用。

Objectives of Study：

To evaluate the efficacy and safety of the method of benefiting qi and activating blood (Shexiang Tongxin Dropping Pills) in the treatment of patients with anthracycline-associated myocardial injuries of the qi deficiency and blood stasis type and to assess its long-term cardioprotective effects.

药物成份或治疗方案详述：

(1)基础用药：规范化西医基础治疗，根据患者实际情况选用右雷佐生、ACEI/ARB、β受体阻滞药和他汀类药物等。 (2)试验用药：麝香通心滴丸，由内蒙古康恩贝药业有限公司生产，国药准字Z20080018，剂型：滴丸，规格：35mg/丸，18丸/板×1板/盒，用量用法：2丸/次，3次/日，口服； (3)麝香通心滴丸安慰剂：由内蒙古康恩贝药业有限公司生产，安慰剂和原药外观、口味、用法用量相同。

Description for medicine or protocol of treatment in detail：

(1) Basic medication: standardized western medicine basic treatment according to the actual situation of patients such as dexrazoxane ACEI/ARB and β-blockers and statins. (2) Test medication: Shexiang Tongxin Dropping Pills produced by Inner Mongolia Kang En Bei Pharmaceutical Co. Ltd State Drug License Z20080018 dosage form: drops specifications: 35mg/pill 18 pills/board × 1 board/box dosage and usage: 2 pills/times 3 times/day taken orally; (3) Shexiang Tongxin Dropping Pills Placebo: produced by Inner Mongolia Kang Enbei Pharmaceutical Co. Ltd the placebo and the original drug have the same appearance flavor dosage and usage.

纳入标准：

(1)年龄18-75岁； (2)接受含蒽环类药物一线化疗的乳腺癌或恶性淋巴瘤患者； (3)基线肌钙蛋白I（cTnI）升高超过参考值上限的99百分位值； (4)中医证候诊断为气虚血瘀证者； (5)自愿签署知情同意书。

Inclusion criteria

(1) Age 18-75 years; (2) Patients with breast cancer or malignant lymphoma receiving first-line chemotherapy with anthracycline-containing agents; (3) Elevated baseline troponin I (cTnI) above the 99th percentile of the upper reference value; (4) Those diagnosed with Qi deficiency and blood stasis by Chinese medicine evidence; (5) Voluntary signing of informed consent.

排除标准：

(1)急性冠脉综合征、心源性休克、难以控制的严重心律失常、急性肺栓塞、高血压危象； (2)具有与原发肿瘤相关的严重并发症、甚至危及生命； (3)1个月内有脑血管、胃肠道、呼吸道、泌尿道或其他器官出血史； (4)患有严重精神类疾病； (5)对研究药物成分过敏； (6)严重感染； (7)妊娠或哺乳期； (8)原因不明的转氨酶持续升高（高于正常上限的3倍）； (9)肾功能衰竭：估计肾小球滤过率＜45 mL/min/1.73 m2； (10)近1个月内参加其他临床试验者。

Exclusion criteria：

(1) Acute coronary syndrome cardiogenic shock uncontrollable severe arrhythmia acute pulmonary embolism hypertensive crisis; (2) With serious complications related to the primary tumor even life-threatening; (3) Have a history of cerebrovascular gastrointestinal respiratory urinary tract or other organ bleeding within 1 month; (4) Suffering from serious mental illness; (5) Hypersensitivity to components of the study drug; (6) Serious infection; (7) Pregnancy or lactation; (8) Unexplained persistent elevation of aminotransferases (greater than 3 times the upper limit of normal); (9) Renal failure: estimated glomerular filtration rate <45 mL/min/1.73 m2; (10) Participation in other clinical trials within the last 1 month.

A block group randomization scheme was used. Within each block group subjects were randomly assigned to either the test or control group in a 1:1 ratio. The randomization sequence was generated by an independent statistician using a computerized random number generator to ensure randomness and unpredictability of allocation.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

本项目的质量控制和保证数据收集和管理将使用纸质病例报告表（CRF）完成。 根据CRF填写标准，数据管理员将原始数据准确、及时、全面、规范地填入CRF。临床监查员将监督试验是否遵循试验方案，并确认所有病例报告表填写符合上述要求。如有错误和遗漏，及时要求研究者更正。对CRF数据的修改必须遵守适用的标准操作规程。修改时需保持原有记录清晰可见，改正处需经研究者重新签名并注明日期。临床试验结束后，CRF将由临床监查员检查数据完整性，然后移交给数据管理员进行录入。对于完成的病例报告表在研究者、监查员、数据管理员之间的传送应有专门的记录，收到时应有相应的签名，记录需妥善保存。电子数据采集（EDC）系统中的电子CRF严格按照CRF的研究方案和内容进行设计。当最后一名患者随访结束时，所有数据将在广州中医药大学第一附属医院以电子方式输入。两组训练有素的研究人员将进行独立的双重数据录入，数据录入结束后应抽查部分观察表格，以确保准确性。数据库支持数据格式、有效值和范围检查。建立数据库后，原始病例报告表和电子数据文件均应分类保存并备份，设置检索目录，以备查考。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Quality control and assurance data collection and management for this project will be accomplished using paper Case Report Forms (CRFs). According to the CRF completion criteria the data manager will fill in the raw data into the CRF in an accurate timely comprehensive and standardized manner.The clinical supervisor will monitor whether the trial is following the trial protocol and confirm that all case report forms are completed in accordance with the above requirements. Any errors and omissions will be promptly requested to be corrected by the investigator. Modifications to the CRF data must be made in accordance with applicable SOPs. Corrections should be made in such a way as to keep the original records legible and the corrections should be re-signed and dated by the investigator. Upon completion of the clinical trial the CRF will be checked for data integrity by the clinical supervisor and then handed over to the data manager for entry. There should be a specific record of the transmission of the completed case report form between the investigator the monitor and the data manager with the appropriate signatures upon receipt and the record should be properly maintained. The electronic CRF in the Electronic Data Capture (EDC) system is designed to strictly follow the study protocol and content of the CRF. All data will be entered electronically at the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine when the last patient's follow-up is completed. Two groups of trained researchers will perform independent dual data entry and a sample of observation forms should be checked at the end of data entry to ensure accuracy. The database supports data formatting validity values and range checking. Once the database is established both the original case report forms and the electronic data files should be categorized and backed up with a searchable directory set up for reference.
