研究目的：

膝骨关节炎（KOA）是一种关节退行性疾病，可导致关节疼痛、畸形与活动功能障碍，会严重影响患者的生活质量，给患者、家庭和社会造成了沉重的负担。“东垣针法”重视调理脾胃，补益后天之本，重用肝脾肾三经的原穴（输穴），以通达三经气机，导气同精，重用土经穴位或各经土穴，以同精导气，平阴阳、调气机。针刺激痛点（MTrP）对缓解KOA患者的疼痛症状、改善膝关节功能具有积极作用。本研究拟在“东垣针法”指导下针刺膝关节周围MTrP用于KOA的治疗，并采用膝关节活动度测量、步态特征分析等客观指标进行临床疗效评估，探讨其可能的作用机制，为KOA的临床治疗提供依据。

Objectives of Study：

Knee osteoarthritis ( KOA ) is a joint degenerative disease which can lead to joint pain deformity and activity dysfunction. It will seriously affect the quality of life of patients and cause a heavy burden on patients families and society. The ' Dongyuan acupuncture method ' attaches importance to regulating the spleen and stomach tonifying the acquired foundation and reusing the original acupoints ( Shu acupoints ) of the three meridians of liver spleen and kidney so as to reach the qi movement of the three meridians guide qi and essence and reuse the soil meridian acupoints or soil acupoints of each meridian so as to guide qi with essence calm yin and yang and regulate qi movement. Acupuncture stimulation of pain point ( MTrP ) has a positive effect on relieving pain symptoms and improving knee joint function in KOA patients. This study intends to use MTrP around the knee joint for the treatment of KOA under the guidance of ' Dongyuan acupuncture ' and to evaluate the clinical efficacy by objective indicators such as knee joint activity measurement and gait characteristic analysis and to explore its possible mechanism of action so as to provide a basis for the clinical treatment of KOA.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

1.确诊为膝关节骨性关节炎，年龄在40-75岁之间；2.Kellgren&Lawrence 放射分级Ⅱ-Ⅲ级；3.接受治疗前，3个月或以上未未接受过相关治疗，如：口服非甾体类镇痛药，关节内注射玻璃酸钠、激素类药物，其他针刺治疗等；4.能接受和坚持针刺治疗4周，积极配合临床数据采集，并签署知情同意书。

Inclusion criteria

1.Diagnosed as knee osteoarthritis aged between 40-75 years old ; 2.Kellgren & Lawrence radiation grade II-III ; 3.Before receiving treatment 3 months or more have not received relevant treatment such as : oral non-steroidal analgesics intra-articular injection of sodium hyaluronate hormone drugs other acupuncture treatment ; 4.Ability to accept and adhere to acupuncture treatment for 4 weeks actively cooperate with clinical data collection and sign informed consent.

排除标准：

1.Kellgren&Lawrence 放射分级＞Ⅲ级；2.合并有强直性脊柱炎、类风湿性关节炎、化脓性关节炎、关节结核、肿瘤及关节内骨折等；3.参加本研究前3个月内接受过其他治疗，如：口服非甾体类镇痛药，关节内注射玻璃酸钠、激素类药物，其他针刺治疗等；4.既往曾接受过关节置换、关节镜等相关外科手术治疗；5.妊娠或哺乳期妇女；6.合并心脑血管、肝、肾、胃及造血系统等严重疾病、精神病患；7.凝血功能异常或者近期在服用阿司匹林、华法林等抗凝血药物；8.存在视觉障碍病、前庭障碍疾病、中枢和周围神经系统疾病以及其他影响平衡功能等的疾病。

Exclusion criteria：

1.Kellgren & Lawrence radiological classification > grade III ; 2.Combined with ankylosing spondylitis rheumatoid arthritis suppurative arthritis joint tuberculosis tumors and intra-articular fractures ; 3.Participated in the first 3 months of this study received other treatment such as : oral non-steroidal analgesics intra-articular injection of sodium hyaluronate hormone drugs other acupuncture treatment ; 4.Previously received joint replacement arthroscopy and other related surgical treatment ; 5.Pregnant or lactating women ; 6.Combined with cardiovascular and cerebrovascular liver kidney stomach and hematopoietic system and other serious diseases mental patients ; 7.Coagulation dysfunction or recent use of aspirin warfarin and other anticoagulant drugs ; 8.There are visual impairment vestibular disorders central and peripheral nervous system diseases and other diseases that affect balance function.

A third party not involved in the trial uses a random number generator (SPSS 25.0) to generate random numbers. According to the random numbers the subjects are randomly assigned to different experimental groups.

盲法：

Blinding：

Blinding is implemented for the included subjects. They will only be told that they will receive one of the two acupuncture intervention measures. Blinding is implemented for data statisticians and analysts. They do not know the grouping situation of the test patients and are only responsible for data statistics and analysis. For the acupuncturists responsible for implementing the intervention it is non-blinded. They must know which intervention measure is used for the test patients so blinding cannot be implemented. However they are not responsible for the statistics and analysis of the trial data.
