通过临床试验，观察针灸推拿治疗痉挛性脑瘫患儿临床疗效

Objectives of Study：

Through clinical study to observe the clinical effect of acupuncture and tuina on children with spastic cerebral palsy.

药物成份或治疗方案详述：

治疗方法： 病例分组： 全部病例按照半随机和患儿家属自愿的原则，分为治疗组和对照组。 对照组治疗方法： 采用Bobath、作业疗法、物理疗法等传统康复方法。由富有经验的康复师根据患者的年龄，身体和运动情况制定个性化的康复方案。每天1次，每周5次，，三个月为一个疗程。 观察组治疗方法： 观察组在治疗组康复方法的基础上采用针灸推拿治疗（基于世中联发布的小儿脑瘫国际中医药临床指南）。 针刺治疗： 头针主穴：运动区、足运感区、平衡区 体针主穴：肝俞、脾俞、太冲、太白、阳陵泉、三阴交 针刺方法：头部穴位沿皮平刺，强刺激，体针太冲施泻法，余穴则多平补平泻，每次留针20分钟，中间行针一次，每天 1 次，治疗 30 次休息一周。 推拿治疗： 手法包括循经推按、穴位点压、异常部位局部肌肉推拿、异常姿势矫正。以轻重有度、先后有 序为推拿手法原则,以柔克刚，以刚制柔为手法准则。 循经推按：上肢沿手三阴经、手三阳经顺序，下肢沿足三阳经、足三阴经顺序，使用推法、按法滚法等复合手法。 穴位点压：对各经脉主要穴位（参照针灸取穴），施以点揉、按压复合手法。 异常部位局部肌肉推拿：用揉法、拿法等舒缓手法。 姿势矫正：对于脊柱及四肢关节的异常姿势，采用扳法、摇法、拔伸法。 操作时按照先上肢、后下肢，每一肢体四种操作按顺序施术。每日 1 次，每次 20～30 分钟，多采用放松类手法，点按穴位参照针刺取穴。

Description for medicine or protocol of treatment in detail：

Therapeutic method: Grouping method: All cases will be divided into Observation group and control group according to the principle of semi random and voluntary of children's families. Treatment of control group: The traditional rehabilitation methods such as Bobath, occupational therapy (OT) and physical therapy (PT) are selectively applied, and the experienced rehabilitation therapist formulates an individualized rehabilitation plan according to the patient's age, physical quality and motor function. Once a day, five times a week for three consecutive months. Treatment of observation group: The observation group are treated with acupuncture and tuina combined with traditional rehabilitation therapy.The traditional rehabilitation methods for observation group are same as the control group(Based on the International Chinese Medicine Clinical Guidelines for Pediatric Cerebral Palsy issued by the World Federation of Chinese Medicine).. Acupuncture treatment: Main points of scalp acupuncture: motor area, foot movement sensation area and balance area. Main points of body acupuncture: Ganshu(BL18), Pishu(BL20), Taichong(LR3), Taibai(SP3), Yanglingquan(GB34) and Sanyinjiao(SP6) Acupuncture methods: For scalp acupuncture, strong stimulation is used, catharsis is used at Taichong(LR3) and even reinforcing-reducing method are used at other points. Acupuncture needles with a diameter of 0.35mm and a length of 25mm shall be selected. The needles shall be retained for 30 minutes, and hand-manipulating of needle shall be once every 15 minutes. Tuina treatment: Manipulation includes tuina along meridians, acupoint pressure, abnormal local muscle tuina, abnormal posture correction. The principle of tuina is light and orderly, and the principle of manipulation is flexible and rigid. Tuina along meridians: Upper limbs along the hand three yin meridians, hand three yang meridians sequence, lower limbs along the foot three yang meridians, foot three yin meridians sequence, using push method, roll method and other composite techniques. Acupoint pressure: Compound techniques such as point, kneading and pressing were applied to the main acupoints of each meridian (Referring to acupuncture). Abnormal local muscle tuina: Soothing techniques such as kneading and holding. Posture correction: The pulling method, shaking method and stretching method are used for the abnormal postures of the spine and limbs, Operation in accordance with upper limbs, lower limbs, each limb four operations in order. Once a day, 20-30 minutes each time. Tuina mostly adopts relaxation manipulation. Treatment duration: Treatments will be conducted every day with the interval of 2 days every week during the weekend. A course of treatment duration is four weeks.

纳入标准： ①符合诊断标准的病人； ②年龄在3-14岁之间； ③自愿配合治疗观察者。

Inclusion criteria

Inclusion criteria 1) The children are diagnosed as spastic cerebral palsy (liver exuberant and spleen weakness). 2) The age of the children is between 3 and 14 years old. 3) The childrens guardian sign the informed consent form.

排除标准：

排除标准： ①不符合上述诊断标准者； ②合并有其他严重的心、肝、肾等重要脏器的器质性疾病者； ③排除脑白质营养不良症、婴儿型脊髓性肌萎缩、脊髓-小脑共济失调综合征、遗传代谢性疾病如苯丙酮尿症以及先天性克汀病、先天性脑积水等现代医学通过病因治疗、手术治疗可获显效的病人； ④非疗效原因的中期中断治疗者； ⑤不配合治疗观察者。

Exclusion criteria：

Exclusion criteria 1) Those who do not meet the above diagnostic criteria. 2) Patients with other serious organic diseases of heart, liver, kidney and other important organs. 3) Exclude leukodystrophy, infantile spinal muscular atrophy, spinocerebellar ataxia syndrome, and hereditary Metabolic diseases such as phenylketonuria, congenital cretinism, congenital hydrocephalus. 4) Those who interrupt treatment in the medium term for non-curative effect reasons. 5) Do not cooperate with the treatment observer.
