研究目的：

主要目的：通过应用“合募配穴”针刺联合营养治疗方法，观察癌性厌食患者进食状况以及生活质量，对该疗法的有效性及安全性进行评价，为临床实践提供确凿证据，助力肿瘤患者厌食症状的科学管理。次要目的：初步探究针刺干预癌性厌食的作用机制，并为针刺干预癌性厌食的机制探究提供临床理论支持。

Objectives of Study：

Primary objective: By applying the therapy of "combination of he-sea and front-mu points acupuncture" with nutrition management and we will observe the eating status and quality of life of patients with cancerous anorexia in order to evaluate the effectiveness and safety of this therapy which aimed to provide conclusive evidence for clinical practice and assisted in the scientific management of anorexia symptoms in cancer patients.Secondary objective: Preliminarily explore the mechanism of acupuncture intervention in cancerous anorexia and provide clinical theoretical support for further exploration of the mechanism of acupuncture intervention in cancerous anorexia.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

（1）患者为III~IV期进展期肿瘤患者；（2）年龄18～85岁；（3）能经口进食，无消化道出血、肠梗阻等进食禁忌证；（4） 食欲视觉模拟评分（VAS）≤70；（5）入组前1个月和入组期间没有接受或计划接受手术、放疗、化疗、免疫治疗的患者；（6） ECOG评分为0-2分；（7）估计生存时间≥6个月；（8）知情同意，志愿受试，知情同意过程符合GCP的规定。

Inclusion criteria

(1) The patient with progressive stage III-IV cancer; (2) Age between 18-85 years old; (3) Able to eat orally without any dietary restrictions such as gastrointestinal bleeding or intestinal obstruction; (4) Visual Analog Scale (VAS) score for appetite is ≤70; (5) No surgery radiotherapy chemotherapy immunotherapy or planned surgery radiotherapy chemotherapy or immunotherapy within the past month or during the trial; (6) The ECOG score is 0 - 2; (7) Estimated survival time is at least 6 months; (8) Informed consent voluntary participation and informed consent process comply with GCP regulations.

排除标准：

（1）患有严重心律失常、严重肝肾功能异常（谷草酰乙酸转氨酶、谷氨酸-丙酮酸转氨酶或总胆红素高于正常3倍或血尿素氮或尿肌酐高于正常2倍）或免疫系统或造血系统疾病的患者；（2）妊娠期、哺乳期妇女；（3）患有凝血障碍、血小板减少症或其他出血性疾病的患者；（4）确诊患有抑郁症、焦虑症和/或精神病的患者；（5）针刺刺激区淋巴水肿患者；（6）害怕针刺刺激或对不锈钢针头过敏的患者；（7）根据研究者的判断、具有降低入组可能性或使入组复杂化的其他情况。

Exclusion criteria：

(1) Patients with severe arrhythmia severe abnormalities of liver and kidney function (glutamic oxaloacetic transaminase glutamic-pyruvic transaminase or total bilirubin more than three times the normal level or blood urea nitrogen or creatinine more than twice the normal level) or diseases of the immune system or hematopoietic system; (2) Pregnant or lactating women; (3) Patients with bleeding disorders thrombocytopenia or other bleeding disorders; (4) Patients diagnosed with depression anxiety and/or psychiatric disorders; (5) Lymphedema patients at the acupuncture stimulation site; (6) Patients who are afraid of acupuncture stimulation or allergic to stainless steel needles; (7) Other situations that in the opinion of the investigator reduce the likelihood of enrollment or complicate enrollment as determined by the investigator.

在常规营养治疗的基础上，给予“合募配穴”针刺治疗。患者取仰卧位，暴露腹部及四肢，针刺胃的合募穴（足三里、中脘），针刺入25-35mm 深度，并通过30s行针手法（平补平泻）获得得气（以患者针刺部位有疼痛、酸胀、麻重感为度），留针30min/次。3 次/周，共治疗4 周。

干预措施代码：

Intervention：

Based on the conventional nutritional treatment the acupuncture therapy will be implemented. We will acupuncture Zusanli and Zhongwan with the insertion depth ranging from 25 to 35 mm. The needling manipulation is conducted for 30 seconds to achieve deqi. The needle will be retained for 30 minutes each time. The treatment is administered 3 times a week for a total of 4 weeks.

在常规营养治疗的基础上，给予患者非穴浅刺。所有非穴点采用经皮浅刺，针尖仅刺入皮下的2-3mm，不施手法，不得气，留针30min/次。3 次/周，共治疗4 周。

Intervention：

Based on the conventional nutritional treatment we will acupuncture at non-acupoints. All the non-acupoints are subjected to percutaneous shallow needling where the needle tip merely pierced 2-3mm beneath the skin. No manipulation was performed and no deqi was achieved. The needles were retained for 30 minutes each time. The treatment was given 3 times a week for a total of 4 weeks.
