研究目的：

优化儿童结肠镜检查肠道准备方案，提高肠道准备质量及患儿肠道准备依从性，降低结肠镜检查非计划延时发生率，提升儿童患者就医体验感。

Objectives of Study：

Optimize the bowel preparation protocol for pediatric colonoscopies to improve the quality of bowel preparation and children's compliance with it reduce the rate of unplanned delays during colonoscopies and enhance the patient experience for young patients.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

年龄3-16岁的除单纯性阑尾炎无其他疾病的儿童；腹部阑尾超声结果阑尾肿大，直径>6mm，阑尾壁厚>0.1cm或阑尾管腔有粪石或脓液形成；肠道准备导泻剂选用复方聚乙二醇电解质散PEG4000；家属及患儿拒绝行外科手术，同意行ERAT术且愿意配合肠道准备。

Inclusion criteria

Children aged 3-16 years with uncomplicated appendicitis and no other diseases; abdominal ultrasound shows an enlarged appendix with a diameter >6mm a wall thickness >0.1cm or fecaliths or pus formation in the appendiceal lumen; for bowel preparation the osmotic laxative chosen is PEG4000; the family and patient refuse surgical intervention and agree to undergo ERAT willing to cooperate with bowel preparation.

排除标准：

结肠镜检查禁忌症或对造影剂过敏者；阑尾穿孔、弥漫性腹膜炎或阑尾周围脓肿者；超声显示腹腔游离气体及泌尿、妇科导致的急腹症；家属拒绝行内镜治疗或不予以配合者。

Exclusion criteria：

Contraindications for colonoscopy or hypersensitivity to contrast agents; patients with perforated appendix diffuse peritonitis or periappendiceal abscess; cases where ultrasound shows free gas in the abdomen or acute abdominal conditions caused by urological or gynecological issues; family members who refuse endoscopic treatment or fail to cooperate.

高剂量分次口服复方聚乙二醇电解质散（PEG）：在计划行ERAT前一天下午6~8点第一次进行口服PEG，检查当天术前4~6h第二次服用，每次服用时间<1h，总量不超过4L，术前2h进行肠道评估，若不合格，则采用改良式灌肠法对患儿的肠道准备进行补救

基于常规肠道准备方法，将喝药量进行细化，3～7岁患儿每15分钟喂服药液250 mI，≥7岁患儿每15分钟口服药液400～500 mI，之后在儿童专属步道进行步行运动15分钟，再对患儿进行清大肠、揉支沟、摩腹、揉龟尾、推下七节骨等穴位按摩15分钟，以喝药—运动—按摩的模式循环进行干预，直至阶段口服药量足够。

Intervention：

Divide the dosage of the medication: for children aged 3 to 7, administer 250 ml of liquid every 15 minutes; for children aged 7 and above, give 400 to 500 ml of liquid orally every 15 minutes. Follow this cycle: take the medicine for 15 minutes, exercise for 15 minutes, and massage for 10 minutes, repeating until the full oral dose is completed.

本实验采用非随机对照试验，根据患者进入研究的先后顺序来分配对照组和干预组，2024年6月至2024年12月来院的所有符合纳排标准的阑尾炎患儿纳入对照组（现已收集108例对照组），2025年1月至2025年6月来院治疗的患儿纳入运动干预联合穴位按摩干预组。

This study employed a non-randomized controlled trial.The control group and the intervention group were assigned based on the order in which the patients entered the study.All children with appendicitis who met the inclusion and exclusion criteria and visited the hospital from June 2024 to December 2024 were included in the control group(a total of 108 cases have been collected for the control group so far).Children who visited the hospital for treatment from January 2025 to June 2025 were included in the group receiving exercise intervention combined with acupoint massage intervention.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：
