将穴位按摩和灸疗相结合，研究穴位按摩联合灸疗在防治肺癌化疗相关性便秘的疗效。

Objectives of Study：

Combining acupoint massage with moxibustion therapy the study aims to investigate the efficacy of acupoint massage combined with moxibustion in preventing and treating CIC in lung cancer patients.

药物成份或治疗方案详述：

1.对照组：常规护理 实施常规护理，便秘的预防工作主要包括以下方面: 1.1责任护士应指导患者养成定时排便习惯，指导患者自行进行有规律的腹部按摩，可顺时针及逆时针按摩腹部，以利于加强肠道蠕动。 1.2指导患者进行日常排便自我观察，对生活可自理者，应鼓励其尽量下床活动，加强肢体功能锻炼，如散步、体操、太极拳等；对于长期卧床，生活无法自理的患者，应教会其通过抬高臀部、收缩腹肌、收缩肛门等运动以增强腹部及肛周肌肉的收缩，増加便意。 1.3加强饮食宣教，忌辛辣、煎炸食物，在患者可以耐受,无心、肾疾患的前提下,鼓励患者增加液体的摄入(温开水效果可能更好)，多吃水果及蔬菜。日常多食用润肠通便的食物，如香蕉、蜂蜜、核桃或酸奶、豆类、洋葱等。饮食可适当结合中医膳食干预，注意饮食有节，起居有常，注重饮食质量。如早餐可以使用郁李仁粥，即15g郁李仁和粳米熬制，具有促进肠胃蠕动，润肠通便和利水消肿的功效。 1.4情志护理：做好心理疏导，减轻患者及其家属的紧张、焦虑心理。 2.试验组1：穴位按摩组 在常规护理的基础上，给予患者穴位按摩，操作者为经过统一培训并通过考核的课题组成员，具体操作流程如下： 2.1穴位: 顺时针摩腹、选取合谷、太渊、尺泽、曲池、中府、天枢、足三里、肺俞、大肠俞穴位按摩。 2.2操作时间： 上午9：00-11：00 2.3手法的运用: 操作时主要采用按揉法，饱餐后和空腹时不宜按揉。遵循“实则泻之”的手法运用原则，在穴位上揉按，频率稍快；在手法力度上以患者能感觉酸、麻、胀、热，又能耐受为宜；对于年老体弱患者，力度宜轻，对于体壮患者，力度稍重；按摩时间约20 min。 2.4操作流程: ①患者取仰卧位，双上肢放松，平放于身体两侧。 ②操作者分别点按合谷、太渊、尺泽、曲池、中府、天枢、足三里、肺俞、大肠俞，每个穴位按揉1min双侧均要按摩，点按后顺时针摩腹。 ③患者每天接受 1 次循经穴位按摩，每次约20 min，连续干预7d 3.试验组2：穴位按摩联合隔姜灸组 在常规护理联合穴位按摩基础上，增加隔姜灸。 3.1 隔姜灸操作流程: 评估：当前主要症状、临床表现及既往史；患者体质及施灸处的皮肤情况；对疼痛的耐受程度；心理状况 告知：治疗过程中局部皮肤可能出现烫伤等情况；艾绒点燃后可能出现较淡的中药燃烧气味；治疗过程中局部皮肤产生烧灼、热烫的感觉，应立即停止治疗；治疗过程中局部皮肤可能出现水疱。 物品准备：治疗盘、艾柱、打火机、弯盘、凡士林、姜片、棉签、针头、镊子，必要时备浴巾、屏风。 基本操作方法: （1）核对医嘱，做好解释，备齐用物，携至床旁。 （2）取合理体位，暴露施灸部位，注意保暖。 （3）遵照医嘱，确定施灸腧穴部位。 （4）施灸腧穴部位（神阙穴）涂少量凡士林，取新鲜生姜，顺其纤维将生姜横向切成3cm×3 cm×0.3cm大小的片状，每片生姜用针灸针刺几个小孔，将制好的生姜片平放在施灸穴位上，尽量使腧穴位置处于生姜片的圆心。取优质艾绒，把艾绒于模具中制成高约2.5cm、底部直径约3cm的三角锥形艾炷，置于姜片中央，以火点燃艾炷顶端，使其缓慢燃烧。待艾炷即将燃尽且患者自觉有灼痛感时移去，换新艾炷再灸。每穴每次灸5壮，以灸处皮肤潮红为宜。 （5）灸疗结束观察局部皮肤红晕，以不起泡为度，防止艾灰脱落。 （6）灸毕，用镊子取出艾柱，放置熄火瓶内，姜片放于弯盘内，清洁局部皮肤。 （7）协助患者衣着，舒适卧位，整理床单位。 （8）清理用物，做好记录并签名。 3.2 隔姜灸部位 神阙穴 3.3隔姜灸频次 每日1次，每次15min 3.4隔姜灸疗程 共干预7天

Description for medicine or protocol of treatment in detail：

纳入标准：

（1）经病理组织学或细胞学检查确诊，诊断为肺部恶性肿瘤患者； （2）符合西医便秘，及中医便秘（虚秘）的诊断标准； （3）经临床诊断与评估，拟在肿瘤科接受化疗治疗，化疗方案含铂类药物的患者； （4）年龄≥18且≤80周岁，性别不限，预计生存期≥３个月； （5）KPS（功能状态Karnofsky评分表）评分≥70分； （6）患者能合作评价便秘症状，经检查无严重肝肾功能不全疾病，无肠道病变； （7）腹部推拿部位皮肤完整；非禁食患者； （8）患者意识清醒,签署知情同意书。

Inclusion criteria

(1) Confirmed by pathological histology or cytology examination diagnosed as patients with malignant lung tumors; (2) Meet the diagnostic criteria for constipation in Western medicine and constipation (deficiency constipation) in traditional Chinese medicine; (3) After clinical diagnosis and assessment patients who are scheduled to receive chemotherapy treatment in the oncology department and whose chemotherapy plan contains platinum-based drugs; (4) Age ≥ 18 and ≤ 80 years old gender not limited with an expected survival period of ≥ 3 months; (5) KPS (Functional Status Karnofsky Rating Scale) score ≥ 70 points; (6) The patient can cooperate in evaluating constipation symptoms and after examination there is no severe liver or kidney dysfunction and no intestinal lesions; (7) The skin at the abdominal massage site is intact; non-fasting patients; (8) The patient is conscious and has signed the informed consent form.

排除标准：

（1）其他原因（局部病变等）所致的便秘，急性肠梗阻、习惯性便秘、使用抗抑郁药等的患者； （2）合并有消化道出血或有其他部位及脏器感染以及中晚期不能耐受治疗病人； （3）肠道造瘘手术后、直肠大部分切除术（＞50%）后患者； （4）合并有心血管、肝、肾、造血系统等严重原发性疾病及精神病患者； （5）化疗期间参加任何其他预防或治疗便秘的临床研究； （6）化疗期间使用吗啡等阿片类镇痛药患者； （7）患者自述艾制品过敏者； （8）妊娠及哺乳期妇女。

Exclusion criteria：

(1) Patients with constipation caused by other reasons (such as local lesions etc.) those with acute intestinal obstruction habitual constipation or those using antidepressants; (2) Patients with gastrointestinal bleeding or infections in other parts or organs as well as those in the middle or late stage who cannot tolerate treatment; (3) Patients after intestinal fistula surgery or after total resection of the rectum (>50%); (4) Patients with severe primary diseases such as cardiovascular liver kidney hematopoietic system diseases and those with mental disorders; (5) Patients participating in any other clinical studies for preventing or treating constipation during chemotherapy; (6) Patients using opioid analgesics such as morphine during chemotherapy; (7) Patients who self-report being allergic to adjuvants; (8) Pregnant and lactating women.

按照样本估算量，将研究对象从1号开始到120号进行编号，采用SPSS24.0软件生成120个随机数字后进行升序排序，第1～40个随机数字进入对照组(常规护理组)，第41～80个随机数字进入试验组1(穴位按摩组)，第81～120个随机数字进入试验组2(穴位按摩联合灸疗组)：在卡片上写下研究对象编号、对应的随机数字及组别，分别放入120个不透光的密封信封中；符合纳排标准的患者按照入院先后顺序随机抽取信封，研究者读取卡片，确定患者的分组并进行相应的干预。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Based on the sample estimation quantity the research subjects were numbered from No. 1 to No. 120. Using the SPSS24.0 software 120 random numbers were generated and sorted in ascending order. The first to the 40th random numbers entered the control group (the conventional care group) the 41st to the 80th random numbers entered the experimental group 1 (the acupoint massage group) and the 81st to the 120th random numbers entered the experimental group 2 (the acupoint massage combined with moxibustion group). On the cards the research subject numbers corresponding random numbers and groups were written down and they were placed in 120 opaque sealed envelopes respectively. Patients who met the inclusion and exclusion criteria were randomly selected from the envelopes according to the order of admission. The researchers read the cards to determine the group of the patients and carry out the corresponding intervention.
