研究目的：

(1) 评估八段锦联合抗阻运动对患有肌少症的老年人的肌肉质量、肌肉力量、体能表现和肠道微生物群的影响；以及 (2) 探索运动通过肠道-肌肉轴改善肌少症的内在机制。

Objectives of Study：

(1) to assess the impact of Baduanjin Combined Resistance Exercise on muscle mass muscle strength physical performance and gut microbiota in old people with sarcopenia; and (2) to explore the underlying mechanisms by which exercise improves sarcopenia via the gut-muscle axis.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

(1)年龄≥ 60 岁的人。 (2)根据亚洲肌少症工作组2019年标准（AWGS 2019）[1]诊断为肌少症的个体： 1）使用生物电阻抗分析法（BIA）测量附着骨骼肌质量指数（ASMI），男性小于 7.0 kg/m2，女性小于 5.7 kg/m2；以及 2）男性手握力（HGS）小于 28 千克，女性小于 18 千克；以及/或 3）6 米步速（GS）小于 1 米/秒。 (3）没有定期锻炼习惯的人（定期锻炼的定义是每周≥3 天进行≥20 分钟的剧烈运动，或每周≥5 天进行≥30 分钟的中等强度运动）。 (4）自愿参加并签署知情同意书的个人。

Inclusion criteria

(1)Individuals aged ≥ 60 years. (2)Individuals diagnosed with sarcopenia according to the Asian Working Group for Sarcopenia 2019 criteria (AWGS 2019): 1)Appendicular skeletal muscle mass index (ASMI) measured using bioelectrical impedance analysis (BIA) is < 7.0 kg/ m2 for men and < 5.7 kg/m2 for women; and 2)Handgrip strength (HGS) is < 28 kg for men and < 18 kg for women; and/ or 3)6-m gait speed (GS) is < 1 m/s. (3)Individuals without regular exercise habits (regular exercise is defined as engaging in ≥ 20 minutes of vigorous physical activity on ≥3 days per week or ≥ 30 minutes of moderate-intensity physical activity on ≥5 days per week). (4)Individuals who participated voluntarily and provided a signed informed consent form.

排除标准：

(1) 有绝对运动禁忌症的人，如深静脉血栓或凝血功能障碍。 (2)有认知或感官障碍，妨碍与研究人员有效沟通的人。 (3)患有影响其完成评估或干预能力的疾病的人，包括肌肉骨骼疾病（如骨折）、心血管疾病（如未控制的高血压、血脂异常、恶性肿瘤病史、中风、急性心肌梗塞、心绞痛、室性心动过速）、脑梗塞、恶性肿瘤或残疾。 (4) 患有传染性皮肤病或伤口未愈合或溃烂者。 (5)过去 3 个月内经常服用以下药物的人：抗血小板药物（如阿司匹林、Aggrenox、噻氯匹定、依替巴肽和替罗非班）、镇痛药、抗凝药、降压药或降脂药。 (6)粪便样本采集前 1 个月内接受过可能影响肠道微生物群的干预措施（如抗生素或益生菌）或有炎症性肠病、胃肠道手术或癌症病史者。

Exclusion criteria：

(1) Individuals with absolute contraindications to exercise such as deep vein thrombosis or coagulation disorders. (2) Individuals with cognitive or sensory impairments that prevent effective communication with the researcher. (3) Individuals with medical conditions that interfere with their ability to complete assessments or interventions including musculoskeletal disorders (e.g. fractures) cardiovascular disorders (e.g. uncontrolled hypertension dyslipidemia history of malignancy stroke acute myocardial infarction angina pectoris ventricular tachycardia) cerebral infarctions malignancies or disabilities. (4) Those with infectious skin diseases or unhealed or ulcerated wounds. (5) Persons who have regularly taken the following drugs in the past 3 months: antiplatelet drugs (e.g. aspirin Aggrenox ticlopidine etipaterol and tirofiban) analgesics anticoagulants antihypertensives or lipid-lowering drugs. (6) Individuals who have received interventions that may affect the gut microbiota (e.g. antibiotics or probiotics) or have a history of inflammatory bowel disease gastrointestinal surgery or cancer within 1 month prior to the collection of the faecal sample.

符合条件的 30 名参与者将被随机分配到两个人数相等的小组中的一个：BDJ-RT 组（15 人）或对照组（15 人）。这一分配将基于一个简单的随机序列，该序列由一名未参与研究的统计学家使用 SPSS 27 版（SPSS, Inc.） 随机过程将按性别和年龄进行分层，以确保各组在主要人口特征方面的平衡。

Randomization Procedure (please state who generates the random number sequence and by what method)：

The 30 eligible participants will be randomly assigned to one of two equal-sized groups: the BDJ-RT group (n=15) or the control group (n=15). This allocation will be based on a simple randomization sequence generated by a statistician not involved in the study using SPSS version 27 (SPSS Inc. USA). The randomization process will be stratified by gender and age to ensure balance between the groups in terms of key demographic characteristics.

盲法：

治疗师将根据各组的具体方案进行干预。除治疗师外，所有其他参与者，包括统计人员、评估人员和数据管理人员，都将对组别分配进行盲测。

Blinding：

Therapists will conduct the interventions according to the specific protocols for each group. In addition to the therapist all other participants including statisticians assessors and data managers will be blinded to group assignment.
