研究目的：

2型糖尿病易并发脑小血管病，增加脑卒中风险。运动可刺激肌肉分泌肌因子，调控脑血管功能，发挥脑血管保护潜力，然在该人群中研究不足。本项目拟探讨24周太极拳训练对这一人群的卒中风险、脑血管功能指标、肌细胞因子、代谢指标等的影响，基于肌-脑crosstalk视阈揭示太极拳发挥脑血管保护效应的关键路径，助力临床应用推广落地。

Objectives of Study：

Type 2 diabetes is prone to complications from small cerebral vessel disease increasing the risk of stroke. Exercise can stimulate muscles to secrete muscle factors regulate cerebrovascular function and exert the potential of cerebrovascular protection. However there is insufficient research in this population. This project intends to explore the impact of 24 weeks of Tai Chi training on stroke risk cerebrovascular function indicators muscle cytokines metabolic indicators etc. in this population. Based on the muscle-brain crossstalk perspective it reveals the key path for Tai Chi to exert its cerebrovascular protective effect and helps promote clinical application.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

　①年龄40~70岁，性别不限； 　②同时符合T2DM和CSVD的诊断标准，头颅MRI可见至少1种CSVD影像学标志物； 　③无严重视听或认知功能障碍，能够配合完成各项检查； 　④四肢健全，无运动禁忌症； 　⑤同意加入本研究并签署知情同意书。 　　同时满足上述条件者方可入组。

Inclusion criteria

① Age 40-70 years old regardless of gender; ② It meets the diagnostic criteria of T2DM and CSVD and at least one imaging marker of CSVD can be seen on skull MRI; ③ There is no serious audio-visual or cognitive impairment and can cooperate with the completion of various examinations; ④ Sound limbs and no contraindications for exercise; ⑤ Agree to join the study and sign the informed consent form. Only those who meet the above conditions can be enrolled.

排除标准：

①认知功能障碍者； ②既往存在颅内大血管病变导致的脑梗死、脑出血等相关脑部器质性病变或神经系统感染性疾病、神经功能缺损者； ③合并糖尿病酮症酸中毒、高渗性昏迷等糖尿病急性并发症患者； ④合并心、肝、肾等严重原发性疾病或恶性肿瘤；　 ⑤患有严重心理疾病或精神障碍患者； ⑥患有影响运动功能的神经、肌肉、关节等疾病； ⑦妊娠期或哺乳期女性； ⑧入组前3个月每周规律运动时间累计超过1.5小时。 凡具备上述任一条件者，均排除出该研究。

Exclusion criteria：

①Cognitive dysfunction; ②Patients with previous cerebral infarction cerebral hemorrhage and other related organic brain diseases or infectious diseases of nervous system and neurological function defects caused by intracranial macrovascular diseases; ③Patients with acute complications of diabetes such as diabetic ketoacidosis and hyperosmolar coma; ④Patients with serious primary diseases or malignant tumors such as heart liver and kidney; ⑤patients with serious mental diseases or mental disorders; ⑥Suffering from nerve muscle joint and other diseases affecting motor function; ⑦Pregnant or lactating women; ⑧The cumulative time of regular exercise per week exceeded 1.5 hours in 3 months before enrollment. Those who meet any of the above conditions are excluded from the study.
