研究目的：

验证肌骨超声引导下超微针刀联合运动靶点灭活筋结法治疗肩周炎的临床疗效及可行性，为基层医院治疗肩周炎提供新的思路及循证医学证据,从而更好地指导临床治疗。

Objectives of Study：

To verify the clinical efficacy and feasibility of the combination of muscle bone ultrasound guided ultramicro needle knife and motion target inactivation method for the treatment of shoulder periarthritis providing new ideas and evidence-based medicine for the treatment of shoulder periarthritis in primary hospitals and better guiding clinical treatment.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

1: 符合中医及西医诊断依据的肩周炎患者 2: 年龄40~70岁，单侧病变，左右不限 3: 能耐受超微针刀治疗的患者 4: 未参与其他临床试验者 5: 签署知情同意书 6: 依从性强，能积极配合治疗，提供完整信息者 诊断标准 1.中医诊断标准 参考2015年中国针灸学会发布的《循证针灸临床实践指南:肩周炎》： (1)气血不足、复感风寒湿邪、慢性劳损、外伤筋骨所致。 (2)慢性发病,好发年龄在50岁左右,右肩多见,常见重体力劳动者。 (3)肩周疼痛,夜间加剧,常因外邪及劳累而诱发,关节活动受限。 (4)肩部肌肉萎缩,有广泛性的触痛,有显著的外展、外旋和后伸功能的限制,“扛肩”现象典型。 (5)X线多为阴性。 2.西医诊断标准 西医诊断标准参考2015年中国针灸学会发布的《循证针灸临床实践指南：肩周炎》中指定的西医诊断标准，结合该标准依据的“Codman五项”，议定如下： (1)发病较为缓慢。 (2)肩部疼痛（肩关节三角肌止点周围），发病初期呈阵发性，随后逐渐加重，夜间加重，气候变化或劳累后加重。 (3)肩关节各个方向主动、被动活动均受限，以外展、上举、内旋、外旋更为明显。 (4)三角肌、冈上肌等肩周肌肉早起可出现痉挛，晚期可发生失用性肌萎缩，出现肩峰突起，上举不利等典型症状。 (5)X线检查多为阴性，病程久者可见骨质疏松。

Inclusion criteria

1: Patients with shoulder periarthritis who meet the diagnostic criteria of traditional Chinese medicine and Western medicine 2: Age 40-70 years old unilateral lesion left or right not limited 3: Patients who can tolerate ultramicro needle knife treatment 4: Participants who did not participate in other clinical trials 5: Sign informed consent form 6: Strong compliance able to actively cooperate with treatment and provide complete information DC 1. TCM diagnostic criteria Refer to the Evidence-based Acupuncture: periarthritis of shoulder issued by the Chinese Society of Acupuncture and Moxibustion in 2015: (1) Lack of qi and blood recovery of wind and dampness chronic strain bone and muscle trauma. (2) Chronic onset the age of the disease is about 50 years old the right shoulder is more common physical workers are more common. (3) Shoulder pain aggravated at night often caused by external evil and fatigue limited joint movement. (4) Shoulder muscle atrophy extensive tenderness significant abduction external rotation and posterior extension limitations typical shoulder resistance phenomenon occurs . (5) The X-ray is mostly negative. 2. Diagnosis criteria for Western medicine The diagnostic criteria for Western medicine refer to the diagnostic criteria for Western medicine specified in the "Clinical Practice Guidelines for Evidence-based Acupuncture and Moxibustion: periarthritis of shoulder" issued by the Chinese Society of Acupuncture and Moxibustion in 2015 and combined with the "five Codman items" based on the criteria it is agreed as follows: (1) The onset is relatively slow. (2) Shoulder pain (around the stop point of the deltoid muscle of the shoulder joint) paroxysmal at the beginning of the disease then gradually aggravated aggravated at night aggravated by climate change or fatigue. (3) Active and passive activities of the shoulder joint are limited in all directions and the abduction upper lifting internal rotation and external rotation are more obvious. (4) Deltoid muscle supraspinatus muscle and other shoulder muscles can appear early spasm late can occur inappropriate muscle atrophy acromion protrusion adverse lifting and other typical symptoms. (5) X-ray examination is mostly negative and osteoporosis can be seen in a long course of disease.

排除标准：

1: 其他疾病引起的肩关节疼痛，如结核、肿瘤、肩部外伤、严重骨质疏松等 2: 伴有精神疾病，无法较好的配合治疗实施或不能连续完成治疗方案者 3: 恐惧抵触针刀治疗者 4: 怀孕妊娠期、哺乳期患者 5: 伴有其他内脏器质性病变（肝腹水、脊柱结核）、凝血功能障碍者（DIC、特发性血小板减少症） 6: 正在接受其他相关治疗，可能会影响本试验观测指标者

Exclusion criteria：

1: Shoulder joint pain caused by other diseases such as tuberculosis tumors shoulder trauma severe osteoporosis etc 2: Individuals with mental illness who are unable to cooperate effectively with treatment implementation or are unable to continuously complete treatment plans 3: Fear of resistance to needle and knife therapy 4: Pregnant and lactating patients 5: Patients with other visceral organic lesions (liver ascites spinal tuberculosis) and coagulation dysfunction (DIC idiopathic thrombocytopenia) 6: We are currently receiving other related treatments that may affect the observation indicators of this trial
