评估经颅重复刺激法联合电针治疗面瘫病的疗效

Objectives of Study：

Evaluate the efficacy of transcranial repetitive stimulation combined with electroacupuncture in the treatment of facial paralysis.

药物成份或治疗方案详述：

针刺治疗方法 （1）取穴：于发病后第3天开始，取百会穴，健侧运动区（相当于大脑皮层中央前回部位，上点在前后正中线中点向后移0.5厘米处，下点在眉枕线和鬓角发际前缘相交处，两点之间的连线即为该区，下2/5是面部运动区），患侧四白穴、阳白穴、下关穴、颊车穴、迎香穴，双侧地仓穴、翳风穴、合谷穴，夹承浆穴。 （2）操作：局部皮肤常规消毒，患者坐位或仰卧位，百会穴采用0.30mm×40mm （华佗牌，苏州针灸用品有限公司生产，苏械注准：20162270588）毫针平刺30mm，施以经颅重复刺激法，即捻转的速度为每分钟200 ~ 300次，持续捻转3 min~5min；取健侧运动区的下2/5处，针尖向眉枕线和鬓角发际前缘相交处的前1.5cm方向平刺 30 mm，手法同百会穴，以局部热、麻、酸胀为得气。阳白穴采用0.30mm×25mm 毫针，针尖向眉骨平刺10mm；四白穴采用0.30mm×25mm 毫针，直刺至神经孔；其余穴位采用0.30mm×25mm 毫针，采用常规针法，刺15mm。 电针：BT 701-1A型电针仪，采用疏密波，连接运动区、阳白穴、四白穴、下关穴、承浆穴、颊车穴、地仓穴（1对）、合谷穴（1对），共5对，强度根据患者的耐受程度和体质以出现麻、刺感为宜，通电30 min。其他穴位常规针刺，留针30 min。 捻针力度：持手拇指、食指紧握针柄，针体在帽状筋膜下层随拇食指前后快速旋转各180°，期间可配合提插手法以催气。要求施术者意、力、气结合而达到针刺刺激的最大力度。

Description for medicine or protocol of treatment in detail：

Acupuncture Treatment Method (1) Acupoint Selection: Start on the third day after the onset of the condition select the Baihui point the motor area of the healthy side (equivalent to the central precentral gyrus of the cerebral cortex with the upper point located 0.5 cm posterior to the midpoint of the anterior-posterior midline and the lower point at the intersection of the brow occipital line and the anterior edge of the hairline at the temple; the line connecting these two points defines the area with the lower 2/5 being the facial motor area) the Sibai point Yangbai point Xiaguan point Jiachuan point Yingxiang point on the affected side and the Dacang point Yifeng point Hegu point on both sides as well as the Jiacheng point. (2) Procedure: Perform routine disinfection of the local skin with the patient in a sitting or supine position. For the Baihui point use a 0.30mm×40mm needle (Hua Tuo brand produced by Suzhou Acupuncture Supplies Co. Ltd. Su Medical Device Approval: 20162270588) for a flat inserti

纳入标准：

纳入标准： ①符合贝尔麻痹的诊断标准； ②首次、单侧发病； ③发病天数≤3天； ④初次就诊时H-B面神经功能等级在Ⅱ～Ⅳ（轻中度）的患者； ⑤就诊前未接受其他相关治疗； ⑥年龄范围在18-80岁之间，精神正常且有生活能力者； ⑦能接受并配合经颅重复刺激运动针法的治疗并自愿在知情同意书上签字者； 同时符合以上标准者，才可纳入本项研究。

Inclusion criteria

Inclusion criteria: 1. Meet the diagnostic criteria for Bell's palsy; 2. First occurrence unilateral onset; 3. Onset within ≤3 days; 4. Patients with H-B facial nerve function grade II to IV (mild to moderate) at the initial consultation; 5. No other related treatments received prior to consultation; 6. Aged between 18 and 80 mentally normal and capable of daily living; 7. Able to accept and cooperate with transcranial repetitive stimulation acupuncture treatment and voluntarily sign the informed consent form; Only those who meet the above criteria can be included in this study.

排除标准：

①面瘫病继发于其他疾病者，如病毒感染、外伤、肿瘤、中耳炎等； ②不符合纳入标准的其他类型的面瘫，如亨特面瘫等； ③伴有系统性疾病如心肾功能异常、高血压、糖尿病等； ④妊娠女性； ⑤凡面部位有严重感染者； ⑥同时参加其他临床实验及无法配合治疗方案安排者。

Exclusion criteria：

1. Facial paralysis secondary to other diseases such as viral infections trauma tumors otitis media etc.; 2. Other types of facial paralysis that do not meet the inclusion criteria such as Hunter's facial paralysis etc.; 3. Accompanied by systemic diseases such as heart and kidney dysfunction hypertension diabetes etc.; 4. Pregnant women; 5. Individuals with severe infections in the facial area; 6. Those participating in other clinical trials and unable to comply with treatment plan arrangements;

本研究采用计算机生成的随机数方法进行随机化，由独立统计人员负责实现，以确保分组的随机性和客观性。具体而言，符合纳入标准的患者在招募后通过在线随机化工具按1:1比例随机分配至经颅重读刺激组和对照组，从而减少组间潜在偏倚，并提供更可靠的研究结果。

This study used a computer-generated random number method for randomization implemented by independent statisticians to ensure the randomness and objectivity of group assignments. Specifically eligible patients were randomly assigned in a 1:1 ratio to the transcranial re-stimulation group and the control group using an online randomization tool after recruitment thereby reducing potential biases between groups and providing more reliable research results.

在本研究中，我们采用了标准化的病例记录表（CRF）和基于互联网的电子数据采集系统（EDC），具体使用ResMan进行数据管理。CRF系统地收集参与者的基础信息、病史和治疗结果，确保数据的准确性。通过ResMan，我们实现了实时数据录入和监控，具备自动校验功能，及时识别数据录入错误。此外，研究遵循相关伦理要求，保障参与者的隐私和数据安全，从而提高了研究数据的完整性和可靠性。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

In this study we used a standardized case report form (CRF) and an internet-based electronic data capture system (EDC) specifically utilizing ResMan for data management. The CRF systematically collected participants' baseline information medical history and treatment outcomes ensuring data accuracy. Through ResMan we achieved real-time data entry and monitoring with automatic validation features to promptly identify data entry errors. Furthermore the study adhered to relevant ethical requirements safeguarding participants' privacy and data security thereby enhancing the integrity and reliability of the research data.
