研究目的：

（1）“怡神养老”推拿能够提高PSCI患者MoCA、MMSE评分，改善认知功能，并可提高MBI指数及SF-36评分，提高患者日常生活能力，改善生活质量。 （2）推拿能够改善PSCI大鼠学习和记忆能力，提高Zea-longa神经功能损伤评分。 （3）推拿能够减小PSCI大鼠脑梗塞灶体积，减轻脑皮质及海马组织病理损伤，降低线粒体损伤蛋白COXIV、SDHA、ROS表达，减轻线粒体损伤。 （4）线粒体自噬推拿能够激活PINK1/Parkin通路，促进线粒体自噬表达，加快线粒体损伤修复。

Objectives of Study：

(1) "Yishen Yangnao" Tui Na can enhance MoCA MMSE scores and cognitive function in PSCI patients. It also boosts MBI and SF-36 scores improving daily living abilities and quality of life. (2) Tui Na improves learning and memory in PSCI rats and raises Zea-longa scores. (3) Tui Na reduces cerebral infarct volume in PSCI rats lessens cortical and hippocampal damage and lowers expressions of mitochondrial injury proteins COXIV SDHA and ROS. (4) Autophagic Tui Na activates the PINK1/Parkin pathway promoting mitochondrial autophagy and accelerating mitochondrial repair.

药物成份或治疗方案详述：

基础治疗：（1）参照《中国脑血管病防治指南》给予相应的卒中后二级预防治疗；针对认知障碍予盐酸多奈哌齐片或盐酸美金刚片服用。（2）受试者均参照《中国脑卒中康复治疗指南》进行常规康复，可根据实际功能缺损情况进行相应的康复训练，其他治疗还包括中药内服、针灸、营养管理及并发症处理等。推拿手法：手法操作部位主要为头枕部、面部、颈部及患侧肢体。具体操作过程：（1）患者仰卧位，术者以拇指按揉法沿后枕部、颈肩部往返操作5-10遍，并重点弹拨枕下肌群、颈椎椎旁肌及横突部，同时拿捏斜方肌、肩胛提肌，同时以指揉法、抹法、一指禅推法操作于头面部，往返操作5-10遍，最后点按百会、四神聪、风池、心俞、内关穴，每穴按压1min，按压力度以患者耐受为度，不可暴力刺激，以上操作时间共约30min，每周6次，连续2周。

Description for medicine or protocol of treatment in detail：

Basic treatment: (1) Secondary stroke prevention follows the "Chinese Cerebrovascular Disease Prevention Guide". For cognitive impairment donepezil hydrochloride or memantine hydrochloride tablets are prescribed. (2) Per the "Chinese Stroke Rehabilitation Guide" routine rehabilitation is done.Other treatments include oral Chinese medicine acupuncture dietary management and complication treatment. Massage: The main areas are the head occiput face neck and affected limbs. Process: (1) Patient supine. Therapist uses thumb kneading on occiput and neck 5 - 10 times focusing on suboccipital muscles cervical paraspinal muscles and transverse processes. Also pinch trapezius and levator scapulae. Use finger kneading wiping and one-finger Zen pushing on head and face 5 - 10 times. Press Baihui Sishencong Fengchi Xinshu and Neiguan for 1 minute each pressure per patient tolerance. Total time: ~30 minutes. Frequency: 6 times per week for 2 weeks.

纳入标准：

（1）符合诊断标准；（2）发病时间为2周-6月；（3）病情稳定，能配合完成量表评定；（4）蒙特利尔认知评估量表（MoCA）评分＜26分；（5）患者及家属知情同意，自愿参加本研究。

Inclusion criteria

(1) Meet the diagnostic criteria. (2) Disease duration of 2 weeks to 6 months. (3) Stable condition and able to complete scale assessments. (4) Montreal Cognitive Assessment (MoCA) score < 26. (5) Informed consent from patient and family for voluntary study participation.

排除标准：

（1）合并其他原因导致认知障碍的相关疾病；（2）6个月内有酗酒、药物滥用、吸毒或相关物质依赖史；（3）存在推拿禁忌或不宜的情况。

Exclusion criteria：

Exclusion Criteria: (1) Cognitive impairment from other causes. (2) History of alcohol abuse drug misuse substance dependence in the past 6 months. (3) Contraindications to massage therapy.

PSCI预后与发病年龄、基础疾病、病灶部位、介入时间等因素有关，因此，本研究拟采用分层随机法，入组分配时实施分组隐匿。本试验干预手段均为外治法，治疗者和患者无法设盲，统计研究人员可设盲。统计分析结束后揭盲。

PSCI prognosis relates to age underlying diseases lesion location and intervention timing. This study will use stratified randomization with concealed allocation. As the intervention is external treatment blinding the therapists and patients isn't feasible but statistical analysts can be blinded. Unblinding occurs after statistical analysis.
