研究旨在探讨中药汤药联合舒筋通络推拿对髋关节骨折老年患者术后功能康复效果的影响。具体目标包括： 评估疗效：通过对比观察组（接受中药汤药和推拿治疗）与常规组（接受常规术后康复干预）患者的功能恢复情况（使用Harris评分）和术后疼痛情况（使用VAS评分），确定联合治疗的有效性。 制定治疗方案：完成中药汤药联合舒筋通络推拿的治疗设计，形成一套有效的治疗体系，为老年髋关节骨折患者术后康复提供参考标准和依据。 发表研究成果：撰写相关论文并在专业期刊上发表，以丰富该领域的研究成果，推动中医药和推拿疗法在髋关节骨折术后康复中的应用。 填补研究空白：通过本研究，弥补目前在老年患者髋关节骨折术后康复领域的研究不足，推动相关理论和实践的发展。

Objectives of Study：

Here is the translation of the specified research objectives: Evaluate Efficacy: To compare the functional recovery of elderly patients with hip fractures between the observation group (receiving traditional Chinese medicine and massage therapy) and the control group (receiving standard postoperative rehabilitation) using the Harris score and pain assessment using the VAS score to determine the effectiveness of the combined treatment. Develop Treatment Protocol: To complete the treatment design for the combination of herbal medicine and massage therapy creating an effective treatment system that provides reference standards and evidence for postoperative rehabilitation of elderly hip fracture patients. Publish Research Findings: To write related papers and publish them in professional journals enriching the research outcomes in this field and promoting the application of traditional Chinese medicine and massage therapy in the postoperative rehabilitation of hip fractures. Address Research Gaps: To fill the current research gaps in the rehabilitation of elderly patients after hip fracture surgery through this study promoting the development of related theories and practices.

药物成份或治疗方案详述：

常规组：（1）术后指导患者下肢行股四头肌等长收缩、踝关节屈伸等；（3）3天后鼓励患者使用助行器下床训练，训练患者坐立等肢体转移训练；（5）患者恢复良好后，练习“金鸡独立”；（6）术后一周，加强臀肌，行走等锻炼，指导家属学习康复技巧和出院后居家功能锻炼方法。 观察组：在常规组康复指导基础上增加中药汤药和舒筋通络推拿，具体如下。 （1）中药汤药：桃红四物汤加减方（桃仁10g，川芎6g，酒当归10g，赤芍10g，三棱6g莪术6g，苏木10g，骨碎补15g，焦六曲10g，牛膝12g，野菊花20g，炒牡丹皮10g,泽兰10g，炒海螵蛸10g，紫花地丁15g，生地黄10g，茯苓10g，甘草6g）100ml，早晚各服用一次。 （2）舒筋通络：在每日在指导患者行功能锻炼前，首先予放松、理筋手法，手法要求轻巧柔和，进行轻度的推摩、擦摩、揉拿等4～6min，先从上向下自股前侧、外侧至后侧；再转自内侧，由下向上：原则上为先轻后重、由慢而快的进行；（2）接着以摆动类、震动类手法为主：予逐渐进行髋前侧及髋后侧拔伸牵引；术后第1天用力宜轻，第2～4d逐渐加力，配合局部点按揉穴位；保持持久、均匀、柔和、有力、深透，维持力量和动作的连贯性，维持有效时间，让手法刺激逐渐积累到较好效应量,连续治疗1周。

纳入标准：

纳入标准：（1）年龄≥60周岁；（2）初次进行全髋关节手术；（3）单侧股骨颈患者；（4）患者无其他严重身体、精神疾病，能够正常进行沟通交流。

Inclusion Criteria: Age ≥ 60 years. First-time total hip surgery. Unilateral femoral neck fracture. Patients without other severe physical or mental illnesses able to communicate normally.

排除标准：

排除标准：（1）年龄小于60岁；（2）双侧股骨颈骨折患者；（3）有髋部手术病史；（4）有严重身体、精神疾病，不能够正常进行沟通交流患者。

Exclusion criteria：

Exclusion Criteria: Age < 60 years. Bilateral femoral neck fractures. History of hip surgery. Patients with severe physical or mental illnesses who cannot communicate normally.

研究将采用计算机生成的随机数字表法进行随机分组。具体步骤:随机序列生成: 由项目负责人李建文医生使用计算机软件（如SPSS或随机数生成器）生成随机数字序列。分组: 根据生成的随机数字，将符合入组标准的患者随机分为两组：常规组: 接受常规术后康复干预。观察组: 在常规组的基础上增加中药汤药和舒筋通络推拿干预。随机化的实施:在患者入组后，使用随机数字表法确定每位患者的分组，确保分组过程的盲法性和随机性，以减少选择偏倚。

Randomization Method: This study will use a computer-generated random number table method for random grouping.Specific Steps:Random Sequence Generation: The project leader Dr. Li Jianwen will generate a random number sequence using computer software (such as SPSS or a random number generator).Grouping: Based on the generated random numbers eligible patients will be randomly divided into two groups: Control Group: Receives standard postoperative rehabilitation interventions. Observation Group: Receives additional Traditional Chinese Medicine decoction and Tui Na therapy on top of the standard interventions. Implementation of Randomization: After patient enrollment the random number table method will be used to determine the grouping of each patient ensuring the blinding and randomness of the grouping process to reduce selection bias.
