研究目的：

本课题将利用脑电能量和脑网络分析方法，研究“醒脑开窍”针刺治疗首发脑卒中和复发性脑卒中患者治疗前后脑电能量及脑网络参数动态变化的不同，评估2组患者针刺治疗的临床及脑电的调控作用，分析脑电能量、脑网络参数与临床评分的相关性，从脑网络重塑方向探索针刺法对首发与复发脑卒中的不同治疗机制，为针灸治疗脑卒中提供电生理机制方面的证据。

Objectives of Study：

This topic will use the method of EEG energy and brain network analysis to study the difference in the dynamic changes of EEG energy and brain network parameters before and after the treatment of the first stroke and recurrent stroke patients treated with "Xingnaokaiqiao" acupuncture evaluate the clinical and EEG control effects of acupuncture treatment of the two groups of patients analyze the correlation between EEG energy brain network parameters and clinical scores explore the different treatment mechanisms of acupuncture on the first stroke and recurrent stroke from the direction of brain network remodeling and provide electrophysiological evidence for acupuncture and moxibustion treatment of stroke.

药物成份或治疗方案详述：

(一) 分组：分为首发卒中组及复发卒中组，每组20人。2组患者均在常规治疗的基础上加以针刺治疗。 (1)研究对象与分组： a. 首发性脑卒中患者：20例，从上海市**医院招募。 b. 复发性脑卒中患者：20例，从上海市**医院招募。 (二)治疗方法： 1.基础药物治疗参照《中国急性缺血性脑卒中诊治指南2018》标准用药。 2. 针刺治疗 2.1.1 针刺治疗时间：每天1次，连续10天为一个疗程。 2.1.2 体针采用石学敏院士所创醒脑开窍针法，并根据辨证分型临证加减(《中国脑梗死中西医结合诊治指南2017》)。 主穴：双侧内关(手厥阴心包经),水沟(督脉), 患侧三阴交(足太阴脾经)； 副穴：患侧极泉(手少阴心经), 患侧尺泽(手太阴肺经), 患侧委中(足太阳膀胱经)； 配穴：吞咽障碍加风池、翳风、完骨；手指握固加合谷；语言不利加廉泉, 金津、玉液放血；足内翻加丘墟透照海。 2.1.3 操作方法： 主穴：先刺双侧内关，直刺0.5 ～ 1寸，采用捻转提插相结合的泻法,施手法1分钟；继刺人中，向鼻中隔方向斜刺0.3 ～ 0.5寸，用重雀啄手法，至眼球湿润或流泪为度；再刺三阴交，沿胫骨内侧缘与皮肤呈45°角斜刺，进针1 ～ 1.5寸，用提插补法，使患侧下肢抽动3 次为度。 副穴：极泉，原穴沿经下移1寸，避开腋毛，直刺1 ～ 1.5寸，用提插泻法，以患侧上肢抽动3次为度；尺泽,屈肘成120°角，直刺0.5 ～ 1寸，用提插泻法，使患者前臂、手指抽动3次为度；委中，仰卧直腿抬高取穴，直刺0.5 ～ 1寸，提插泻法，使患侧下肢抽动3次为度。 以上穴位均留针20分钟后起针。

Description for medicine or protocol of treatment in detail：

Acupuncture treatment 2.1.1 Acupuncture treatment time: once a day for 10 consecutive days as one course of treatment. 2.1.2 The body acupuncture adopts the method of awakening the mind and opening the orifice created by Academician Shi Xuemin and is modified according to the syndrome differentiation and clinical conditions ("Chinese Guidelines for the Diagnosis and Treatment of Cerebral Infarction with Integrated Traditional Chinese and Western Medicine 2017"). Main acupoints: Bilateral Neiguan (Hand Jueyin Pericardial Meridian) Shuigou (Du Mai) and San Yin Jiao (Foot Taiyin Spleen Meridian) on the affected side; Secondary acupoints: Jiquan on the affected side (Hand Shaoyin Heart Meridian) Chize on the affected side (Hand Taiyin Lung Meridian) and Weizhong on the affected side (Foot Taiyang Bladder Meridian); 2.1.3 Operation method: Main acupoint: First prick the bilateral internal guan with a straight needle of 0.5-1 inch using a combination of twisting turning lifting and inserting for 1 minute; In the process of stabbing someone stab them diagonally towards the nasal septum by 0.3-0.5 inches using the technique of heavy pecking until the eyeballs become moist or shed tears; Further prick the San Yin Jiao diagonally prick along the inner edge of the tibia at a 45 ° angle to the skin insert the needle 1-1.5 inches and use the lifting interpolation method to make the affected lower limb move 3 times as degrees. 纳入标准：

1.诊断标准:参照《中国急性缺血性脑卒中诊疗指南2018》，急性期经头颅CT扫描或核磁共振(MRI)证实为脑梗塞者； 2. NIHSS评分：4分---15分； 3.年龄18岁以上；病程在7天内； 4.意识清楚，生命体征平稳，无心、肝、肺、肾等严重并发症，能够配合治疗；

Inclusion criteria

1. Diagnostic criteria: Referring to the "2018 Guidelines for the Diagnosis and Treatment of Acute Ischemic Stroke in China" patients with cerebral infarction confirmed by head CT scan or magnetic resonance imaging (MRI) in the acute phase; 2. NIHSS score: 4-15 points; 3. Age over 18 years old; The course of the disease is within 7 days; 4. Clear consciousness stable vital signs no serious complications such as heart liver lungs kidneys etc. able to cooperate with treatment;

排除标准：

1.短暂性脑缺血发作； 2.所取穴位处有感染、溃烂、湿疹等不适合行针者。

Exclusion criteria：

1. Transient ischemic attack; 2. If there is infection ulceration eczema or other conditions at the selected acupoint that are not suitable for acupuncture.
