研究目的：

旨在通过比较子午流注经络按摩、瑞士球及子午流注经络按摩联合瑞士球这三种不同康复锻炼方式对脑卒中偏瘫患者下肢康复锻炼效果，观察探讨三种不同的康复锻炼方式对脑卒中偏瘫患者下肢肌力改善和平衡障碍的影响，为临床上选择制定康复锻炼方案提供新的思路。

Objectives of Study：

The purpose of this study is to compare the effect of three different rehabilitation exercises on the lower limb rehabilitation exercise of stroke patients with hemiplegia by comparing the meridian massage of midnight-noon ebb-flow Swiss ball and meridian massage of midnight-noon ebb-flow combined with Swiss ball and to observe the effect of three different rehabilitation exercises on the improvement of lower limb muscle strength and balance disorder in stroke patients with hemiplegia so as to provide new ideas for clinical selection of rehabilitation exercise programs.

纳入标准：

①西医诊断标准：符合《中国各类主要脑血管病诊断要点 2019》诊断标准，经头颅 CT 或头颅 MRI证实为脑卒中； 中医诊断标准：据1995年国家中管局中医脑血管病急症协作组《中风病诊断与疗效评定标准（试行）》：病名诊断标准：主症：偏身不遂，偏身感觉异常，言语困难或者不能言语，意识模糊，口舌歪斜。次症：眩晕、头痛，瞳孔变化等，饮水困难、呛咳，偏盲，共济运动失调等。起病的方式：大都发病较急，发病之前常有明显诱因，出现先兆症状。发病的年龄：发病年龄多大于40岁。 注：符合２个主症及以上，或符合１个主症伴２个次症及以上，并结合出现的先兆症状、诱因、起病方式、发病年龄即可确诊；对于不符合上述条件的患者，影像学检查结果也可确诊。 ②年龄30-80岁；1个月＜病程＜12 个月；病灶为单侧（陈旧性或未引起肢体活动障碍的梗死也可纳入）；生命体征稳定；监护下能独立站立 10 分钟及以上；可独立行走10-15米；Brunnstrom 分期大于 IV 期；签署知情同意书。

Inclusion criteria

The diagnostic criteria of Western medicine : meet the diagnostic criteria of ' 2019 diagnostic points of various major cerebrovascular diseases in China ' and confirmed as stroke by cranial CT or cranial MRI ; diagnostic criteria of traditional Chinese medicine : According to the 1995 national administration of traditional Chinese medicine cerebrovascular disease emergency cooperative group ' stroke diagnosis and efficacy evaluation criteria ( Trial ) ' : disease name diagnostic criteria : main symptoms : hemiplegia hemidysesthesia speech difficulties or inability to speak blurred consciousness tongue skew. Secondary symptoms : vertigo headache pupil changes difficulty in drinking water cough hemianopia ataxia etc. The way of onset : most of the onset is acute there are often obvious inducements before the onset and there are aura symptoms. Age of onset : the age of onset is more than 40 years old. Note : In line with 2 main symptoms and above or in line with 1 main symptom with 2 secondary symptoms and above and combined with the emergence of symptoms incentives onset age of onset can be diagnosed ; for patients who do not meet the above conditions imaging findings can also be diagnosed. 2 aged 30-80 years old ; 1 month < course of disease < 12 months ; the lesion was unilateral ( old or infarction without limb movement disorder can also be included ) ; stable vital signs ; can stand independently for 10 minutes or more under monitoring ; can walk independently 10-1

排除标准：

严重认知障碍；听觉障碍、视觉障碍；贫血；完全性失语、经皮质感觉性失语；抗凝治疗失衡；癫痫；肝功能，肾功能等减退或衰竭、NYHAIV 级心力衰竭，心房颤动；其他下肢功能障碍及影响步行的疾病（如手术、骨折、严重骨关节炎、关节炎或中重度疼痛）。

Exclusion criteria：

Severe cognitive impairment ; hearing impairment, visual impairment ; anemia ; complete aphasia, transcortical sensory aphasia ; imbalance of anticoagulant therapy ; epilepsy ; liver function, renal function decline or failure, NYHA class IV heart failure, atrial fibrillation ; other lower limb dysfunction and diseases affecting walking ( such as surgery, fracture, severe osteoarthritis, arthritis or moderate to severe pain ).

本研究确立样本量共132例，由第三方通过应用SPSS25.0软件编程进行随机化分组。首先按入组的先后顺序将132例患者编号为1～132，预先规定第一组患者序号为（1～33），第二组患者序号为（34～66），第三组患者序号为（67～99），第四组（100～132）。程序运行后，产生132个随机数字。选用统一的不透光的信封进行随机分配隐藏，由不参与本研究的人员将写有132个随机数字的纸条装进信封内，打乱后在信封外写上编号1～132。按照入组的先后顺序，从编号为１的信封依次往后拆开，研究者按信封内的分组数字对研究对象进行相应的干预并做好记录。

Randomization Procedure (please state who generates the random number sequence and by what method)：

This study established a total of 132 samples, which were randomly grouped by a third party through the application of SPSS25.0 software programming. Firstly, 132 patients were numbered 1-132 according to the order of enrollment. The number of patients in the first group was ( 1-33 ), the number of patients in the second group was ( 34-66 ), the number of patients in the third group was ( 67-99 ), and the number of patients in the fourth group was ( 100-132 ). After the program runs, 132 random numbers are generated. A unified opaque envelope was selected for random allocation hiding. The staff who did not participate in this study put the paper with 132 random numbers into the envelope, and numbered 1-132 on the outside of the envelope after being disrupted. According to the order of enrollment, the envelope numbered 1 was disassembled in turn, and the researchers intervened and recorded the subjects according to the grouping numbers in the envelope.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

１）分别于患者入组当天填写一般资料调查表，内容包括年龄、性别、婚姻状况、文化程度、病程等。 ２）指导患者每日填写康复日记，并于干预前１天、干预3周后、干预6周后填写Berg平衡量表、计时起立行走测试、Fugl-Meyer下肢评定量表、重心轨迹长度、左/右体重分配差异、前/后体重分配差异等。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1 ) The patients were asked to fill in the general information questionnaire on the day of enrollment, including age, gender, marital status, education level, course of disease, etc. 2 ) To guide patients to fill in the rehabilitation diary every day, and to fill in the Berg balance scale, timed up and go test, Fugl-Meyer lower limb rating scale, center of gravity trajectory length, left / right weight distribution difference, front / back weight distribution difference 1 day before intervention, 3 weeks after intervention and 6 weeks after intervention.
