研究目的：

采用随机对照试验明确揿针促进儿童腹腔镜急性阑尾炎切除术后胃肠功能恢复的疗效，为揿针临床治疗提供高质量证据。

Objectives of Study：

To evaluate the effect of thumbtack needling on gastrointestinal function recovery after laparoscopic appendectomy in children with acute appendicitis by a randomized controlled trial so as to provide high-quality evidence for clinical treatment of thumbtack needling.

药物成份或治疗方案详述：

术后胃肠功能障碍是儿童阑尾切除术后的常见并发症。急性阑尾炎且需要行腹腔镜急性阑尾炎切除手术的术后患儿，按照随机编码进入试验组和对照组。试验组采用揿针联合标准护理治疗，刺激穴位取双侧足三里、内关，留针24 h后更换揿针，确保每次针刺区域一致性，嘱患儿依次按压揿针针帽3次/d，3min/次，共干预3d。对照组采用假揿针联合标准护理治疗，取双侧足三里、内关，将一次性假揿针置于穴位，留针24 h后更换假揿针，确保每次针刺区域一致性，嘱患儿依次按压假揿针针帽3次/d，3min/次，共干预3d。

Description for medicine or protocol of treatment in detail：

Postoperative gastrointestinal dysfunction is a common complication after appendectomy in children. Postoperative children with acute appendicitis and in need of laparoscopic acute appendicitis resection were randomly assigned to the experimental group and the control group. In the experimental group press-needle therapy combined with standard care was adopted. The acupoints of bilateral Zusanli and Neiguan were stimulated. The press-needles were retained for 24 hours and then replaced to ensure the consistency of the acupuncture area each time. The children were instructed to press the press-needle caps in sequence 3 times a day for 3 minutes each time with a total intervention of 3 days. In the control group sham press-needle therapy combined with standard care was used. The disposable sham press-needles were placed at bilateral Zusanli and Neiguan and retained for 24 hours and then replaced to ensure the consistency of the acupuncture area each time. The children were instructed to press the sham press-nee

纳入标准：

① 年龄在6-12岁的住院患儿，不限性别。 ② 均经同一组普外科医师根据患儿的临床症状、体征、试验室检查影像学检查结果确诊为小儿急性阑尾炎。 ③ 符合腹腔镜急性阑尾炎切除手术治疗指征。 ④ 患儿及监护人认知功能正常，了解术后可能出现的情况，理解医生关于术后切口瘢痕满意度调查的内容，并配合完成相关调查。 ⑤ 患儿及监护人了解手术治疗的利弊，患儿及监护人均签署知情同意书。

Inclusion criteria

① Children aged 6-12 admitted to the hospital regardless of gender. ② All were diagnosed with acute appendicitis by the same group of pediatric surgeons based on the child's clinical symptoms physical examination findings laboratory test results and imaging examination results. ③ They met the indications for laparoscopic acute appendicitis resection. ④ The child and the guardian have normal cognitive function and understand the possible postoperative situation. They understand the content of the doctor's postoperative scar satisfaction survey and cooperate to complete the relevant survey. ⑤ The child and the guardian understand the advantages and disadvantages of surgical treatment and both sign the informed consent form.

排除标准：

① 超龄患儿。 ② 既往有开腹手术病史者。 ③ 转为开放性手术。 ④ 与POGD无关的患儿。 ⑤ 需要术后重症监护超过24小时。 ⑥ 有晕厥或癫痫病史。 ⑦ 在一个月内服用影响胃肠道功能的药物。 ⑧ 过去3个月内参加其他临床研究或一个月内接受针灸的患儿也被排除在外。 符合以上任何一项者即予排除。

Exclusion criteria：

① Children over the age limit. ② Children with a history of open surgery. ③ Converted to open surgery. ④ Children unrelated to POGD. ⑤ Children requiring postoperative intensive care for more than 24 hours. ⑥ Children with a history of fainting or epilepsy. ⑦ Children who have taken medications that affect the stomach and intestines within the past month. ⑧ Children who have participated in other clinical studies within the past 3 months or have received acupuncture within the past month are also excluded. Anyone who meets any of the above criteria will be excluded.

本研究由独立的第三方研究人员采用 SPSS 25.0 统计软件实现分层区组随机抽样，设区组长度为 4（为保证随机化的实施， 区组长度在临床试验结束前不公开）。根据 SPSS 软件生成的观察序列号及其对应的组别，制成随机分配卡，装入不透光的信封中，将观察序列号作为信封的序号贴至封面，密封备用。将密封的信封交予分中心专人保管负责，各中心按照竞争入组 法进行 1:1 随机化分组，规定受试者的纳入先后顺序即为观察序列号，根据受试者的观察序列号拆开对应编号的信封，提取相应的分组组别，产生试验组和对照组，并严格按照随机分配卡提示的分组结果进行干预。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Stratified block random sampling was performed by an independent third party investigator using SPSS 25.0 statistical software with a block size of 4 (the block size will not be disclosed until the end of the trial to ensure the implementation of randomization). According to the observation serial number and its corresponding group generated by SPSS software a random allocation card was made and put into an opaque envelope. The observation serial number was pastthe cover of the envelope as the serial number and sealed for use. The sealed envelopes were handed over to the special personnel of the sub-centers and the centers were enrolled according to the competitionAccording to the observation serial number of the subjects the corresponding numbered envelopes were opened and the corresponding groups were extracted to produce the experimental group and the control group. The intervention was carried out in strict accordance with the grouping results suggested by the random allocation card.
